- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation VA258-13-Q-0369 is issued as a request for quotation. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. (iv) This procurement is being issued using "Cascading Procedures." Reference the below listed Evaluation Addendum detailed in paragraph ix(b), for complete guidance on order of preference. Accordingly, all business concerns may submit offers for this requirement, but they will only be considered if and when the category of preference above your firm has been eliminated from consideration. The NAICS code is 238160 with a small business size standard of $14M. (v) CLIN 0001, "Install Gutters and Down Spouts" (vi) The Contractor shall supply all labor, tools, materials, equipment, supplies, transportation, management and all other related elements necessary to complete the services entitled "Electronic Statement of Conditions (eSOC)" at the Phoenix VA Health Care System, VA Medical Campus, Phoenix, Arizona, in accordance with the Statement of Work and industry/ commercial standards. Federal, State, and local requirements apply and are the responsibility of the contractor to ensure appropriate compliance. Contractor shall Conduct an assessment addressing the level of life safety compliance of all patient care buildings at the Carl T. Hayden VA Medical Center, Phoenix, Arizona with the using the latest edition of the Life Safety Code (NFPA 101). This is for a period of one year with the option of 4 one year extensions, as follows: 1. Annual site visit to perform the Statement of Conditions (SOC) inspection in compliance with the most recent edition of the Life Safety Code and prepare a compliance document. The last tri-annual TJC SOC survey was conducted in FY 2011. a. Conduct preliminary drawing review and Basic Building Information (BBI) drawing setup b. Conduct a survey of Buildings, 1, 16, 21 and 8 to determine compliance with appropriate editions of NFPA 101, Life Safety Code. This will also include the new Building #31 (Rehabilitation) which is scheduled for completion spring 2013.pancy classification for each section/floor of each building. c.Evaluate the existing fire and smoke barrier walls and make recommendations to decommission the walls and cross corridors walls that are no longer required, or new ones required due to building functions/activities. d.Conduct random survey (10) of fire and smoke barrier walls above the accessible ceilings. (due to infection control concerns, will not survey above ceiling in patient occupied rooms). On redlined drawings will denote wall deficiencies for future repair. e.NFPA deficiencies will be identified and documented so that maintenance related work order and Plan(s) for Improvement (PFI) can be developed. 2.Provide Fire Protection Engineer support for drawing review, NFPA/Joint Commission review for ongoing operations and training. 3.The government reserves the right to revise the number of buildings that are included in the annual SOC inspections, item #1 above. 4.The assessment will also include an evaluation of the following documents: a. Current Joint Commission Statement of Conditions (SOC) including Basic Building Information (BBI), BBI drawing and Plan for Improvement (PFI) b.Life Safety Management Plan (EC5.10) c.Fire protection system, inspection, testing and maintenance documentation (EC.5.40) d.Interim Life Safety Measures (ILSM) procedures (EC.5.50) 5.Must have Auto CAD capabilities for the integration of LSC information into the architectural drawings of the facility. Provide small scale floor plan drawings, in color, showing the current room layout with room numbers, fire walls, smoke barrier walls, smoke compartment names or designations and occupancy classification for each floor for every building current or future. Also identify suites, hazardous rooms and key note any special life safety features. 6.Make recommendations on how to correct any non-compliant items that are not correctable by simple, straight forward methods. 7.Prepare a preliminary PFI in the draft submittal for any items that are not correctable under the work order system. After receiving the medical center's input, complete the PFI for inclusion in the final SOC documents. 8.Provide one master and two numbered copies of the SOC documents. Also provide computer CD containing the final SOC documents and drawings. 9.Provide data entry for the eBBI & ePFI 10.Provide Equivalency preparation and documentation 11.Provide SOC/eSOC Maintenance & Follow-up 12.Plan review & Healthcare Construction Consultation 13.Provide an exit conference after all surveys are completed with Phoenix VAMC Safety, Engineering, and Leadership staff to discuss findings and trends. 14.A draft document will be provided to the Phoenix VAMC within 30 days of the site survey. 15.Provide NFPA consultation on any matter during the term of this contract. 16.Work will be performed in a functioning medical center. The contractor will be using ladders to access areas above the ceilings to observe the integrity of smoke walls throughout the facility. The contractor will not have access to the VA computer system. If the contractor needs to be in a patient sensitive area then arrangements will be made to inspect the area after hours (escorted) or the staff in those areas will be notified within 24 hours prior to the contractor entering the area. In addition the contractor will be escorted by assigned staff. Reference: February 8, 2007 "Contractors Excepted for Background Investigations and Screenings" memorandum from Deputy Under Secretary for Health for Operations and Management. The Phoenix VAMC will provide the contractor with a CD containing the most current electronic drawing files (in Auto CAD) and current fire and smoke barrier wall arrangements for review prior to the contractor's survey. END of SOW (vii) Period of Performance: 30 Calendar days. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix (a)) 52.212-2, Evaluation -- Commercial Items applies. Quotes will be evaluated based on prices offered from qualified vendors who have performed the subject work. (ix (b)) Addendum to 52.212-2 Evaluation-Commercial Items (JAN 99) - Method of Award - "Cascade Procedure" - As follows: 1. Any award(s) resulting from this solicitation will be made using the following cascading order of precedence: 1.1. In accordance with FAR Subpart 19.14 and PL 109-461, award under this solicitation will be made on a competitive basis first to an eligible Service Disabled Veteran Owned small business (SDVOSB) concern (see Section K, FAR 52.219-1, "Small Business Program Representations" for small business size definition) who submits a responsive offer, provided that there is adequate competition among such firms. 1.2. If there is inadequate competition for award to a SDVOSB concern, award will be made competitively to an eligible Veteran Owned Small Business (VOSB) concern. 1.3. If there is inadequate competition for award to a VOSB concern, award will be made competitively to a certified HubZone small business concern. 1.4. If there is inadequate competition for award to a HubZone concern, award will be made competitively to a Women-Owned small business concern. 1.5. If there is inadequate competition for award to a Women-owned concern, award will be made competitively to a small business concern IAW FAR 19.5. 1.6. If there is inadequate competition for award to a small business concern, award will be made on the basis of full and open competition considering all responsive offers submitted by responsible business concerns. 2. Adequate competition shall be deemed to exist if- 2.1 At least two competitive offers are received from qualified, responsible business concerns at the tier under consideration; and 2.2. Award will be made at fair market prices as determined in accordance with FAR 19.202-6. 3. When there is inadequate competition at any tier, an otherwise competitive offer from a responsible business concern at that tier will be considered with the eligible offers for the next lower tier. For example, only one competitive offer is received from a SDVOSB business. That offer will be included with the offer(s) received from all VOSB businesses. If there is still inadequate competition, the offer from the SDVOSB business will then be included with SB offer(s) received; finally, if there is inadequate competition at the SB tier, the SDVOSB offer will be included with all responsive offers. 4. Award will be made to the offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. 5. Pricing will be firm fixed price for each line item identified.
