DISCLAIMER This is a SOURCES SOUGHT NOTICE for market research purposes only to determine the availability of potential businesses with capabilities to provide the services described below. Potential offerors are invited to provide feedback via e-mail to Dean Umathum at Dean.Umathum@va.gov. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. 0001 - Calibration Services for 149 pieces of equipment for the Seattle VA Medical Center. The intended contract period is a one-year base period plus (4) four one-year option periods. Potential contractors shall provide, at a minimum, the following information to Dean Umathum at Dean.Umathum@va.gov 1) Company name, address, point of contact name, phone number, e-mail address, and DUNS. 2) Is your firm eligible for participation in one of the following small business programs? Provide any additional feedback that you feel is relevant (i.e., problems or any other issues experienced with similar contracts). Include comments on the Draft SOW. 5)How are Calibration Services as described in the draft SOW typically priced? How could it be priced for a Government contract? 6) Provide a brief capability statement (Max 2 pages) with enough information to determine if your company can meet the requirement. The Capabilities Statement for this sources sought is not expected to be a Request for Quotations, Request for Proposals or Invitation for Bids, nor does it restrict the Government to an ultimate acquisition approach, but rather the Government is requesting a short statement regarding the company's ability to provide Calibration Services near Department of Veterans Affairs, VA Puget Sound Healthcare System, 1660 South Columbian Way, Seattle WA 98108. Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement. Submission of capabilities statement will assist our office in tailoring the requirement to be consistent with industry standards. The capabilities will be evaluated solely for the purpose of determining to Set-Aside for the Small Business (SB) Community or to conduct as an Unrestricted Procurement. Other than small businesses may respond to this notice in the event the market does not indicate SB interest. This synopsis is for information and planning purposes only and is not to be construed as a commitment by the Government. The Government will not pay for information solicited. Respondents will not be notified of the results of the evaluation. All interested contractors should notify this office via email by 4:00 PM Pacific Daylight Time on March 8th, 2013. Submit response and information to: Dean umathum, Department of Veterans Affairs, Puget Sound Health Care System, Dean.Umathum@va.gov The DRAFT Statement of Work follows 1.Specific Tasks a.Identify all personnel that will be involved in the services performed on the contract and submit the employees to the Seattle VAMC to receive flash badges. b.Contractor shall schedule a maintenance period with the Seattle VAMC to calibrate all equipment listed in the attached. c.Contractor shall calibrate all equipment to manufacturer specifications and standards. d.Contractor shall provide a repair quote for each individual piece of equipment that does not pass calibration tests. e.The Contractor shall generate individual service stickers to be affixed to each piece of equipment calibrated stating the date of calibration, the next calibration due date, and the technician that calibrated it. 2.Deliverables: a.Contractor shall provide a service report, in an electronic or paper format, to the Seattle VAMC. A separate individual service report shall be established for each piece of equipment and shall include i.Serial Number ii.Model Number iii.Description/Nomenclature iv.Calibration Status v.Repair Quote (If applicable) 3.Due Dates a.All equipment shall be calibrated with deliverables received by the Seattle VAMC prior to June 28th 2013. b.This contract has option years. In the event an option year is exercised, all future calibration services along with deliverables shall be completed no later than June 28th of the year an option is exercised. 4.Performance Monitoring The contractor's staff performing the services outlined in this contract will be monitored directly by the Seattle Biomedical Department Staff. The Biomedical department will ensure the contractor is following hospital procedures, performing quality work/inspections, and that the proper calibration documentation happens. 5.Other Pertinent Information a.All expenses including but not limited to travel, hotel, flight, etc. shall be at the contractor's expense. b.Any equipment that cannot be calibrated onsite and needs to be shipped to a different location to perform the calibration shall be at the expense of the contractor. c.All calibration services performed need to be traceable to the National Institute of Standards and Technology (NIST). 6.Technical Prospective contractors, or their employees performing the work outlined in this contract, shall have at a minimum: a.Technician must be certified to calibrate each type of equipment and provide proof of certification. b.At least 3 years of experience successfully calibrating similar types of equipment. See attached document: S02 Calibration Requirement List for a complete list of equipment needed to be calibrated. Thank you for your participation.
