Please contact Greg Guay, Glenmont Purchasing Agent, if you will be in attendence. a) Contractor shall visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted with ID #GL 0001 will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in the performing same. II. Project Description Scope of Project: To Install Life Safety type items. Items that include replacement of exit signs, CO Alarms with battery backup, Replace Fire alarm panel, electrical upgrades to include wire guards to emergency lighting fixtures and the replacement of a 400A, 120/208V, 3-Phase,4 wire main building distribution panel. The following deficiencies are included in this project: 91916 Building Lacks Carbon Monoxide Alarms 91918 Building Lacks Carbon Monoxide Alarms 91919 Building Lacks Carbon Monoxide Alarms 91920 Building Lacks Carbon Monoxide Alarms 91921 Building Lacks Carbon Monoxide Alarms 91809 Fire Alarm Devices and Exit Signs not Adequately Protected 91979 Fire Alarm Enunciator Panel is Deteriorated 91773 Main Distribution Panel Board completely Deteriorated 91810 Exit Sign is Deteriorated 91835 Exit Sign is Deteriorated 91845 Exit Sign is Deteriorated 91782 Exit Sign is Deteriorated 91811 Emergency Egress Lighting Inadequate 91834 Emergency Egress Light Fixture Deteriorated 91832 Combination Emergency Light Exit Sign Deteriorated 91844 Emergency Egress Light Fixture Deteriorated and Removed 91801 Emergency Egress Light Fixture Deteriorated 91793 Remote Emergency Light Head are not Operational and Deteriorated 91804 Emergency Egress Lighting Inadequate 91805 HID Light Fixtures lack Quartz Re-Strike Feature 50232 Fire Alarm - Vocation Building Bathrooms - #0016 Note: A packet containing the exact location of all 21 project deficiencies will also be given out to all at the pre bid walk thru which explains in detail the work to be completed. III Specific Requirements: The center has been funded for replacement of a 400A, 120/208V, 3-Phase, 4 wire main building distribution panel located in the basement of the Administration building. The center's main fire alarm annunciator panel is obsolete, deteriorated and needs to be replaced with a new unit. All emergency light fixtures, exit signs, audio/visual devices and pull stations do not have suitable wire guards to protect fixtures and fire alarm devices from accidental contact or breakage in the gym location. At the vocational building the men and women toilet rooms do not have any audio/visual devices to provide notification signals for the occupants to evacuate the facility, as required by ADA. The Food Service, Gym, Auto, CTS and Welcome Center Buildings all contain fuel using equipment and appliances with Carbon Monoxide (CO) as a by-product. A cracked heat exchanger or failed venting of the equipment or appliances could allow CO to enter the air supplied to occupied spaces. The buildings all require 120V single -station CO alarms with battery backup and integral detection. Six different buildings located at Glenmont are in need of replacing the existing deteriorated exit signs; there are a total of 17 signs that will need to be replaced. Warrantees: •a) The contractor is to provide written warranty for the above statements at time of completion of job IV Construction Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. Management of Jobsite: The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. The Contractor shall clear all deliveries with the Center's security personnel. Wage Rates: The bidders shall conform to the latest Davis-Bacon prevailing wage rate decisions for the area. (See the attached wage rate) ELEC0236-001 06/01/2012 ALBANY; COLUMBIA; FULTON; GREENE (that portion North of a line following the south limits of the city of Catskill); MONTGOMERY; RENSSELAER, SARATOGA; SCHENECTADY; SCHOHARIE; WASHINGTON Rates Fringes ELECTRICIAN......................$ 33.50 3%+19.02 Insurance: The Contractor shall obtain and maintain insurance as follows: Workman's Compensation/Employers Liability insurance, to meet New York statutory requirements Comprehensive General Liability ($1,000,000 per occurrence) Comprehensive Automobile Liability ($1,000,000/combined single limit) Fire Legal Liability ($500,000 per occurrence) The Contractor must cover any deductible costs. Upon contract award, the Contractor must add Glenmont Job Corps academy as additional named insured to the policies. Schedule: The Contractor shall submit a schedule for the work. The project shall be completed within 30 calendar days of the award of the contract. Written authorization is required to extend the 30 day completion requirements. Final Acceptance The Contractor shall notify the Center's facility manager when the project is substantially complete, and shall request an inspection. The completion of this inspection shall determine the date for the commencement of the warrantees, and the date for payment. V. Proposal Form The Contractor's proposal shall be submitted on company letterhead, must reference the proposal ID # and address all items in section III, to be considered. The Contractor shall submit the following information with the proposal: A list of materials and product data sheets (MSDS) proposed for use.
