- AIRMAN MINISTRY CENTER COORDINATOR INTERVIEW QUESTIONS AIRMAN MINISTRY CENTER COORDINATOR STATEMENT OF WORK AIRMAN MINISTRY CENTER COORDINATOR SCORING CRITERIA FOR RATED QUESTIONS This is a combined solicitation/synopsis for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number FA4608-13-Q-S077, in accordance with FAR Parts 12 and 13, and is in accordance with provisions and clauses effective in Federal Acquisition Circular (FAC 2005-64 effective 18 Jan 2013). The North American Industry Classification System (NAICS) number is 561110, and the business size is $7 M. This proposed acquisition is set-aside for small businesses. Small businesses, woman-owned, and disabled veteran-owned businesses are encouraged to participate by submitting quotes. Please see the attached RFQ for further details. The commodity being procured is one (1) Airman Ministry Center Coordinator for one (1) Base Year and four (4) Option Years based on 2080 hours per year: 0001 - Airman Ministry Center Coordinator (Base year) 1001 - Airman Ministry Center Coordinator (1st Option Year) 2001 - Airman Ministry Center Coordinator (2nd Option Year) 3001 - Airman Ministry Center Coordinator (3rd Option Year) 4001 - Airman Ministry Center Coordinator (4th Option Year) *Statement of Work Attached. Offeror's must comply with all instructions contained in FAR 52.212-1, Instructions to Offeror's Commercial Items, applies to this acquisition. Offeror's shall submit their quote on letterhead stationery, provide unit prices with a grand total, and the contractor must remit a completed copy of the REPRESENTATIONS AND CERTIFICATIONS located at FAR 52.212-3, and the 252.212-7000 Offeror Representations and Certifications with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition. The following additional FAR clauses cited in the clause are applicable to this acquisition: 52.219-6, Notice of Total Small Business Aside, Alternate I 52.219-28, Post Award Small Business Program Representation 52.222-3, Convict Labor 52.222-19, Child Labor-Cooperation with Authorities and Remedies 52.222-21, Prohibition of Segregated Facilities 52.222-26, Equal Opportunity 52.222-36, Affirmative Action for Workers with Disabilities 52.225-13, Restrictions on Certain Foreign Purchases 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration THE GOVERNMENT WILL NOT ACCEPT INCOMPLETE PROPOSALS. Offers will be evaluated on the prices as offered. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. If an offer cannot comply with every requirement, that offer will not be responsive, and therefore will not be considered. 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (I) PRICE (II) TECHNICAL ACCEPTABILITY Proposals will be evaluated on a "Lowest Price Technically Acceptable" basis. -Technical acceptability includes Qualifications outlined in section 4 of the attached Airman Ministry Center Coordinator Statement of Work (SOW). This will be validated by a required submission of a resume, which is required to be submitted at time of proposal in accordance with section 4.5 of the SOW. The total for CLINs 0001, 1001, 2001, 3001 and 4001 will be added to arrive at a total price. The offer with the lowest total price will then be evaluated for technically acceptability. If the lowest priced offer is considered technically acceptable, then award shall be made to the low offer and evaluations shall stop at this point. If the low offer is not considered technically acceptable, then the next lowest offer shall be evaluated. This process shall continue until lowest priced technically acceptable offer is evaluated. - An interview will be required before any decision of hiring.** ** The interview process will be held within 5 days after solicitation closing date at Barksdale AFB Chapel 1 between the hours of 8:00a.m. and 4:00 p.m. Interviews will be either face to face or through teleconference. Face to face interviews are preferred but not mandatory. Other written material may be submitted to support oral presentation, but no media will be accepted. The evaluation criteria for the interview questioning process are outlined in the attached "Scoring Criteria for Rated Questions". ONLY the RATED QUESTIONS will determine technically acceptable based on the quantitative results. In addition, the interview will be a maximum of one hour. Options: The Government will evaluate offers for award by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). Quotes can be faxed (in accordance with FAR provision 52.214-13 TELEGRAPHIC BIDS) or emailed to the attention of Richard Compton at (318) 456-2629 NO LATER THAN 24 April 2013 at 12:00pm (CST). Offers received after this date and time will be considered as late submissions in accordance with 52.212-1(f) and will not be evaluated or considered. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
