Description
Notice of Intent to Sole Source Contract The Southern Regional Region Contracting Office, on or about, 15 April 2013 intends to issue a sole source purchase orders for a base and one 12-month option period to Home Intensive Care Pharmacy for the purchase of compound pharmaceuticals for the Pharmacy Department, San Antonio Military Medical Center. Vendor must have The Joint Commission Accreditation and possess the appropriate licensure. This notice is neither a request for quotes nor a solicitation of offers; however, all responsible sources may submit a capability statement, proposal, or quotation, which shall be considered by the agency. Salient Characteristics: - Pharmacy must be accredited by The Joint Commission - Must provide delivery within 30 Minutes - Must be prepared using sterile preparation techniques - Pharmacy must be certified and possess a sterile USP 797 compliant clean room for high risk compounds for preparation - BAMC Burn Ointment must consist of Bacitracin 250 units/gm, Polymyxin B 5,000 units/gm and Nystatin 50,000 units/gm in White Petrolatum, NF and packaged in an Unguator dispensing 1 lb jar and should have a 180 day potency period. - Glutamine 2.5% should contain 2.5 grams of Glutamine per 100 ml and be packaged in a sterile IV bag and should have a 30 day potency period. - Ephedrine injection 5mg/ml 10ml should be prepared and prefilled in a sterile 10 ml syringes and color coded to differentiate between the Ephedrine and the Phenylephrine syringes and should have a 90 day potency period. - Phenylephrine 40 mcg/ml 10ml should be prepared and refill in a sterile 10 ml syringe and color coded to differentiate between the Phenylephrine and the Ephedrine syringes and should have a 90 day potency period. - House Mix Ophthalmic Drops must consist of Tropicamide 1% and Phenylephrine 5% with Edetate Disodium as a buffer and Benzalkonium Chloride as a preservative and should have a 90 potency period. - Progesterone 100 mg suppositories must consist of 100 mg of Progesterone in each suppository and have a 180 potency period. - Benzocaine/Lidocaine/Tetracaine/DMSO consists of Benzocaine 0.2 grams per gram or 20%, Lidocaine 0.06 grams per gram or 6%, Tetracaine 0.04 grams per gram or 4%, and Dimethyl Sulfoxide (DMSO) 0.2ml per gram or 10%, and packaged in an EMP jar and should have a 180 day potency period. - Glycerin/Lidocaine/Epinephrine consists of Lidocaine & Epinephrine 1%, 0.3ml per ml, and Glycerin Injection 70%, 0.7mls per ml, and packaged in sterile vials that should be kept refrigerated and has a 30 day potency period. - Boric Acid (ear capsules) consists of boric acid.0146 grams per cap and packaged in #4 size clear gelatin caps that has a 180 day potency period. - Dexamethasone 24mg Preservative Free injection consists of dexamethasone sodium phosphate 0.024g per ml in sterile water for injection that is packaged in sterile empty vials that should be kept refrigerated and has a 30 day potency period. - Chlor/Sulf/Amp/HCl caps consists of Chloramphenicol 0.05gm or 5% per cap, Sulfanilamide 0.05gm or 5% per cap, Amphotericin B 0.005gm or 0.5%, Hydrocortisone Acetate 0.001gm or 0.1% per cap packaged in #3 white capsules and has a 180 day potency period.
