Description

Clarifies pesticide licensing and endorsement requirements April 11, 2013 Amendment #01 to Solicitation This amendment clarifies the pesticide licensing required to complete this project work. Standard Form (SF) 30 has been attached to this solicitation and states: Possession of licenses and endorsements for the application of carbaryl is required under this solicitation and any resulting contract. All applicators in a commercial crew are required to have a pesticide license with the forestry endorsement in order to apply carbaryl (per Utah Department of Agriculture and Food). All offerors are required to sign/date the SF30 form, acknowledging their receipt of this clarifying information, and return this form along with any offer submitted. The solicitation period has not been extended.
