- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W912EQ-13-Q-0038 and is being issued as a Request for Quote (RFQ). This acquisition is being issued as UNRESTRICTED. The associated NAICS code is 811310. All responsible sources may submit a quote, which shall be considered by the agency. Quotes may be submitted in hard copy or electronically, either through email, or by facsimile. Regardless of method of delivery, it is the offeror's responsibility to ensure receipt of quotes by the time and date stated herein. Contractors shall submit a lump sum quotation on company letterhead to include all charges for the below listed items to be delivered in accordance with the specifications listed below: DESCRIPTION/SPECIFICATIONS: Contractor shall repair and furnish all labor, materials, tools, equipment, services, transportation, taxes, temporary equipment, structure and payment of all cost incidental and necessary in order to complete the work required for the repair/service of a Mack Semi-Truck - Asset: 2006 Mack CT713 - Serial #: 6M003791 - VIN#: 1M2AL02YX6M003791 - Engine: E7 - Engine Hours: 7383 - Miles: 91,554. The following description of work needed for the Mack Semi- Truck includes but is not limited to: Engine is hard to start, primer pump leaking, leak at steering gear box, brake treddle valve leaking, water leak near oil filter stand, front engine seal leaking, front and rear torsion rods need replaced, replace all brakes and s-cam bushings, check engine light is staying on, malfunction light is staying on, transmission will not stay in high gear, head lights daytime running lights and some marker lights are inoperable, bolt broken in head at exhaust manifold, cover missing on fuse box, clamp broken on left side of intercooler hose, top radiator hose leaking, bottom missing on A/C control panel, replace trunnion bushings and end pads, step on left side needs replaced, front pinion seal leaking, main drive belt needs replaced, right rear tail light not working, replace 5th wheel mounting rails - not reinstalling fenders. DELIVERIES Deliveries - Quick turnaround is necessary in order for the Government to fulfill its mission. Therefore, all work must be completed within 30 days after award and must be coordinated with the following personnel: Primary - Hugh Green 901-573-2521 cell hugh.d.green@usace.army.mil SUBMISSION REQUIREMENTS Contractors shall submit quote on company letterhead to include pricing for the requested service, with its unit price, extended price, total price, prompt payment terms, remittance address, Tax Identification Number, DUNS number, and Cage Code. Contractor shall submit completed clause 52.212-3, Offerors Representation and Certifications, Commercial Items.
