Proposal Information Project: Asphalt paving is deteriorated in several areas at Glenmont Owner: US Department of Labor (DOL) Employment and Training Administration Center Operator : Adams and Associates, Inc Mailing address 822 River Road, Glenmont, New York 12077 Proposal Due Date: 5 PM EST May 10, 2013 Evaluation Criteria: Cost Schedule Evaluation of adequacy of system proposed Financial strength of contractor to replace equipment if it does not Meet specification Payment: The Center will make payment within 30 days of completion. Site Visit Date: May 2, 2013 at 9:30 AM. Please contact Greg Guay, Glenmont Purchasing Agent if you will be in attendance. a) Contractor shall visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted with ID #GL0002 will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in the performing same. This proposal shall be valid for a period of 90 Days. II. Project Description Scope of Project: The Asphalt paving is deteriorated in several areas. Areas that include the cafeteria loading dock area, a portion if the loop road due east of Building #12 (Large Chapel) and the parking lot due north of building #5 (Education). In addition the center has several pot whole areas in need of attention beginning from the area near the Welcome Center and proceeding up the hill ½ mile to the vocational buildings. III. Specific Requirements: Remove the damaged surface and base in affected areas. Trim and compact the sub grade. Apply a bonding asphalt coat to vertical surfaces. Backfill holes with plant- mix asphalt concrete... Provide re-grading and rip rap in eroded road shoulders. Warrantees: •a) The contractor is to provide written warranty for the above statements at time of completion of job IV Construction Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. The center will provide a copy of the rules and regulations to the contractor. Management of Jobsite: The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. The Contractor shall clear all deliveries with the Center's security personnel. Wage Rates: The bidders shall conform to the latest Davis-Bacon prevailing wage rate decisions for the area. The project shall be completed within 45 days of the award of the contract. Written authorization is required to extend the 45 day completion requirements. Final Acceptance The Contractor shall notify the Center's facility manager when the project is substantially complete, and shall request an inspection. The completion of this inspection shall determine the date for the commencement of the warrantees, and the date for payment. V. Proposal Form The Contractor's proposal shall be submitted on company letterhead, must reference the proposal ID # and address all items in section II and III, to be considered. The Contractor shall submit the following information with the proposal: A list of materials and product data sheets (MSDS) proposed for use.
