- Past Performance Form Wage Determination No: 1991-0101 Wage Determination No: 2005-2279 Solicitation Document Solicitation Number: AG-569R-S-13-0020 Notice Type: Combined Synopsis/Solicitation Synopsis: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, #AG-569R-S-13-0020, is issued as an RFQ (Request for Quotations) and the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular No. 2005-66. This requirement is for the United States Department of Agriculture, Forest Service. The NAICS code for this project is 336611, Maintenance and repair of ship and marine equipment, and the Business Size Standard is 1000 employees. The intent of this contract is to provide towing service to and from the shipyard, dry-dock and hull inspection, sand blast and painting, stability and load line testing, and other shipyard general services for USFS owned Barge. (See below chart and attached solicitation document for more details). ITEM NO. DESCRIPTION UNIT QUANTITY TOTAL 1 Towing Service; round trip between Munising, MI and Shipyard. LS 1 2 Dry-Dock and Hull Inspection LS 1 3 Sand Blast and Paint LS 1 4 Stability and Load Line tests LS 1 5 Shipyard General Services (Crain Use, etc.) LS 1 6 *Total using Standard 60 Day Performance Period TOTAL 7 *Total using Optional 30 Day Performance Period TOTAL Due to the critical need of this barge during summer months, the Forest Service requests the potential cost variation for a quicker, 30 day performance period. Contractors may bid on one of both of the proposed performance periods however, the government has the right to determine which proposal and performance period is the best benefit to the government. Provisions, clauses, and addenda including, but not limited to, commercial clauses 52.212-1 Instructions to Offerors - Commercial Items (Feb 2012), 52.212-2 Evaluation-Commercial Items (Jan 1999), 52.212-3 Offeror Representations and Certifications - Commercial Items (Dec 2012), 52.212-4 Contract Terms and Conditions - Commercial Items (Feb 2012), and 52.212-5 Contract terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Jan 2013), apply to this contract and are incorporated in the attached solicitation document. The attached past performance & experience form or any other preferred format may be used to address the past performance/experience portion of this criteria. If an offerors quote does not contain all of the items listed above, the quote may be considered incomplete for evaluation purposes and no further consideration will be given to the quote, thereby making an offeror ineligible for award.
