- Attachment 4 - Provisions and Clauses Attachment 3 - Wage Determination Attachment 2 - PWS Attachment 1 - Evaluation Factors FA8601-13-R-0048 - Combined Synopsis Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with (IAW) the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This acquisition uses simplified acquisition procedures IAW FAR Subpart 13.5--Test Program for Certain Commercial Items. This announcement, with its attachments, constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Requirement: The United States Air Force School of Aerospace Medicine (USAFSAM) Center for Sustainment of Trauma and Readiness Skills (CSTARS) requires contracted lodging in the Cincinnati Ohio area. Services are to be provided in accordance with Performance Work Statement dated 5 April 2013, which is Attachment 2 to the attached solicitation document. Solicitation Number: The solicitation number for this requirement is FA8601-13-R-0048. Set-Aside: This is a Small Business Set-aside. The applicable North American Industry Classification System (NAICS) Code is 721110 - Hotels (except Casino Hotels) and the size standard is $30M. Provisions and Clauses: Applicable provisions and clauses are incorporated into the attached solicitation document. Federal Acquisition Regulation (FAR) provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-66, Effective 1 April 2013. Defense Federal Acquisition Regulation Supplement (DFARS) provisions and clauses are those in effect through DFARS Publication Notice (DPN) 20130412, Effective 12 April 2013. Air Force Federal Acquisition Regulation Supplement (AFFARS) provisions and clauses are those in effect through Air Force Acquisition Circular (AFAC) 2013-0327, Effective 27 March 2013. Contract Line Items (CLIN): Detailed CLIN descriptions are provided in the attached solicitation document. Instructions for Submission of Quotes/Offers: Instructions for submission of proposals are contained in the provision at FAR 52.212-1-Instructions to Offerors-Commercial Items, which is incorporated in full text into the solicitation document. Evaluation Factors: Evaluation factors and a detailed description of how proposal will be evaluated are contained in the attached solicitation document, Attachment 1, Evaluation Factors. Consent to RFP Terms and Conditions: Submission of proposals in response to this RFP constitutes agreement by the offeror to all terms and conditions contained in this announcement and the attached solicitation document, which will be incorporated into the resulting contract. Due Date and Point of Contact: Questions in response to this RFP must be received in writing and e-mailed to rachael.waggoner@wpafb.af.mil no later than 3:00 PM, Eastern Standard Time on 1 May 2013. Questions will be responded to no later than 3:00 PM, Eastern Standard Time on 8 May 2013. Proposals must be received no later than 3:00 PM, Eastern Standard Time on 22 May 2013.
