THIS IS NOT A SOLICITATION ANNOUNCEMENT. THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. THE PURPOSE OF THIS ANNOUNCEMENT IS FOR MARKET RESEARCH PURPOSES ONLY. RESPONSES TO THIS ANNOUNCEMENT WILL BE USED BY THE GOVERNMENT TO MAKE APPROPRIATE ACQUISITION DECISIONS. A SUBSEQUENT SOLICITATION ANNOUNCEMENT MAY FOLLOW. RESPONSES TO THIS ANNOUNCEMENT WILL NOT BE CONSIDERED AS AN ADEQUATE RESPONSE TO ANY FUTURE ANNOUNCEMENT. The Louis Stokes Department of Veterans Affairs Medical Center (LSDVAMC), Cleveland, Ohio 44106 anticipates award of an indefinite quantity FIRM FIXED PRICE contract. The purpose of this contract is to engage a community provider (Contractor) who will provide community-based, residential environments, providing sufficient services to meet the needs of the Veterans while they undergoing Intensive Outpatient Program (IOP) Substance Abuse treatment at local VA Community Based Outpatient Clinics (CBOC). Entities that are interested in providing Veterans these services should be aware that this contract seeks services for the area of Mahoning County of Ohio only. The Contractor shall provide housing, to include, but not necessarily limited to beds, restroom, bath and kitchen facilities and related furnishings. The Contractor shall provide food for a minimum of two balanced meals daily and snacks daily to each resident. The Contractor shall provide personal hygiene items and storage as needed. The primary goal is to develop alternative housing for the Veterans and to prepare them for independent living. It is the ultimate goal that these Veterans will eventually find their way to housing located within the community. Through this announcement, the VA seeks to support Veterans served by IOP by expediting placement of these Veterans into safe, supportive housing. This housing must meet the criteria established both by IOP clinical staff and VA fire and safety officials in addition to the local city, county and state regulations. Interested parties should provide the following: Company Name, Company Address, Point of Contact, (POC) phone number/email address, DUNS number and capability statement for the services as stated above. Responses to this notice should include three customer references to which you have provided similar services to, copies of applicable licensing, and the size of your business pursuant to the following questions: (1) Is your business large or small (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified hub zone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small or Veteran Owned Small Business? Those wishing to identify their capability to provide the afore-mentioned service should respond to this announcement via email to Glen.Johnosn@va.gov no later than April 26, 2013 4:00 p.m. Eastern Time. THIS REQUEST FOR INFORMATION, OR "SOURCES SOUGHT," SHALL NOT BE CONSTRUED AS A REQUEST FOR PROPOSALS OR OFFERS. INFORMATION IS TO BE PROVIDED ON A VOLUNTARY BASIS. NO ENTITLEMENT TO PAYMENT OF DIRECT OR INDIRECT COSTS OR CHARGES INCLUDING POSTAGE AND SHIPPING WILL ARISE AS A RESULT OF THE SUBMISSION OF INFORMATION.
