- REQUEST FOR INFORMATION, SOURCES SOUGHT THIS IS A REQUEST FOR INFORMATION (RFI)/SOURCES SOUGHT ONLY AND YOUR RESPONSE IS NOT AN OFFER AND DOES NOT COMMIT THE GOVERNMENT TO PAY ANY COSTS INCURRED IN PREPARATION OF ANY SUBMISSION TO THIS MARKET RESEARCH, OR TO CONTRACT FOR SUPPLIES/SERVICES. The VISN 19 Rocky Mountain Network Contracting Center, 4100 E. Mississippi Ave, Glendale CO is performing market research to determine if there are a sufficient number of qualified Small Business firms to set aside a Blanket Purchase Agreement. The Government will use responses to this notice to make an appropriate acquisition decision. This requirement is for Clinical Hematology Analyzers to conduct a variety of tests on Cost-Per-Reportable-Results. The selected firm shall provide each participating facility with Food And Drug Administration (FDA) approved analyzer/equipment, reagents, controls (both whole blood and body fluid), calibrators, disposables, and consumable part (inclusive of replacement parts per manufactures maintenance guidelines, cap piercing needles, specimen racks, sample holders, etc.), necessary for analyzing/testing on both the primary and backup analyzers. All linearity material and any commercial system correlation samples needed shall be provided to meet CAP and the Joint Commission requirements for the following VA medical facilities: Grand Junction VAMC Colorado, Salt Lake City VAMC Utah and Cheyenne and Sheridan VAMC Wyoming, Fort Harrison and Billings VAMC Montana. The capability statement should be sent to the Contract Specialist Kim Cooper no later than 5:00 p.m. (Mountain Time), 26 April 2013. Solicitation VA259-13-R-0512 for Hematology Cost-Per-Reportable-Result (CPRR) has been assigned and should be referenced on all correspondence regarding this announcement. THIS IS NOT A REQUEST FOR PROPOSAL. NAICS Code 334516 is applicable to this acquisition; the size standard for a firm to be considered under this NAICS Code is a business size of 500 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System for Award Management (SAM) database prior to award of a contract.
