Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number ASAP3282013 is hereby issued as a Request for Quote (RFQ) for the purchase of Individual Synthetic Cannabinoid Metabolites Screenings in support of the U.S. Army Alaska Alcohol and Substance Abuse Program. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-66, effective 28 February 2013. This acquisition will be solicited as quote mark Full and Open Competition quote mark. Therefore, all vendors under NAICS code 621511 Medical Laboratories are eligible to compete. The intent of the Government is to establish a Blanket Purchase Agreement with a qualified vendor IAW FAR Part 13.303 and FAR part 12. This Blanket Purchase Agreement does not obligate any funds. Funds are obligated when an order is place under this agreement. The Blanket Purchase Agreement is anticipated for a total period of five years. Description of Requirement: CLIN 0001 Synthetic Cannabinoid Screen (Urine)QTY 1$________________ The Synthetic Cannabinoid Screen should at a minimum consist of the following tested metabolites. However, due to the dynamic nature of cannabinoid development, testing requirements may evolve to meet new requirements. AM-2201 N-(4-hydroxypentyl) metabolite JWH-018 N-(4-hydroxypentyl) metabolite JWH-018 N-(5-hydroxypentyl) metabolite JWH-019 N-(5-hydroxyhexyl) metabolite JWH-019 N-(6-hydroxyhexyl) metabolite JWH-073 N-(3-hydroxybutyl) metabolite JWH-073 N-(4-hydroxybutyl) metabolite JWH-081 (O-desmethyl N-4-hydroxypentyl) JWH-081 (O-desmethyl N-5-hydroxypentyl) JWH-122 N-(4-hydroxypentyl) metabolite JWH-122 N-(5-hydroxypentyl) metabolite JWH-210 N-(4-hydroxypentyl) metabolite JWH-210 N-(5-hydroxypentyl) metabolite JWH-250 N-(4-hydroxypentyl) metabolite RCS-4 (O-desmethyl N-4-hydroxypentyl) metabolite UR-144 N-(5-hydroxypentyl) metabolite UR-144 N-pentanoic acid metabolite Vendors must submit a sample report outlining cut-off levels used to identify presence of me-tabolites, and address possible inaccuracy rates per metabolite. All screenings will be conducted via Urine sample. The government will be responsible for monitoring and establishing initial chain of custody on each sample specimen. The government will be responsible for collection, packaging, and ensuring specimens are re-leased to a vendor supplied shipping company (Fedex/UPS) for delivery to testing location. Shipments will originate from Fort Wainwright, Alaska (Fairbanks, AK) and Joint-Base Elmen-dorf-Richardson, Alaska (Anchorage, AK). The vendor will supply all necessary supplies to include collection bottles, seals, chain of custo-dy forms, shipping containers w/postage, and any other supplies needed for the execution of this requirement. (The vendor will be responsible for all shipping charges related to this re-quirement.) Vendor must submit a copy of standard testing procedures with their correspondence. Vendor will be responsible for testing and reporting results within 7 days of receipt of samples. All positive samples will be verified with a second confirmation test and stored for a minimum of 6 months to allow support of legal proceedings. Vendor must be capable of providing lab report and Expert Witness testimony in regards to Military Courts Martial proceedings. Vendor must submit a copy of their Licensure and Accreditation with their correspondence. All vendors are required to be registered in the System for Award Management (SAM) and have access to Wide Area Work Flow (WAWF) for billing.
