- Sources Sought: This is a correction to a previous sources sought announcement for Jefferson County. The Size standard for NAICS code 621498 was incorrectly stated as $10 million. Correct size standard for 621498 is $19 million. Vendors who have previously replied to the sources sought need only send an email stating their business size based on the correct size standard. Below is the corrected Sources sought notice. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 621498, Size Standard $19 Million. The Network Contracting Office at Syracuse (VISN 2) is seeking a potential qualified contractor to provide primary care and certain specialty care in a Capitated Community Based Outpatient Clinic (CBOC) setting in Jefferson County in the state of New York. Responses to this sources sought notice must include the following information: (1) General information about your company including name, address, contact person, phone number, fax number, email address(s), DUNS number and company website if available. (2) Type of business such as 8(a), small business, Hub Zone small business, Small Disadvantaged business, Service Disabled Veteran Owned small business, Veteran Owned small business, large business. (3) Does your company have experience with the Department of Veterans Affairs capitated CBOC Program? (4) What supplies/services does your company primarily provide? (5) Will your company be a joint venture with another company? (6) How long has your company existed? Responses to this market survey should be in writing and shall be faxed (315-425-4883) to the attention of Lisa Tompkins, Contract Specialist or emailed to Lisa.tompkins@va.gov. No telephone responses will be accepted. Responses must be received no later than 3:00 pm April 30, 2013. No solicitation document is available at this time; this notice is to acquire information only. Vendors interested in providing an offer will have to respond to a solicitation announcement that shall be published separately.
