- Wage Determination WD 05-2047 (Rev.- 13) Performance Work Statemente (PWS) U.S. DEPARTMENT OF DEFENSE U.S. AIR FORCE-SMC/PKO REQUEST FOR QUOTE FA2816-12-T-0007 A. GENERAL INFORMATION SMC/PK 61st ABG/HC has a requirement for a new Catholic Parish Administrator procurement. This is a streamlined combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only notice. A written solicitation will not be issued. This requirement is issued as a "Request for Quote". Award will be made to the contractor who meets the specifications set forth in the Performance Work Statement (PWS) of this solicitation in accordance to guidance from the Department of Labor, Wage Determination List (Service Act of 1965 - (SCA). Past Performance will be evaluated based on relevance of the work described in the Performance Work Statement (PWS) and recency; work performed within the last three years. Past Performance and Price will have equal weight in the evaluation process. This proposed contract is being considered as a set-aside under the small business set-aside program. The NAICS Code is 611710 and the Size Standard $7M. The government is interested in all small businesses that are interested in performing this requirement. A Firm Fixed Priced award will be made in accordance with the tentative schedule described in the Description section of this solicitation. 1. DESCRIPTION/QTY: The Contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items of services necessary to provide a part time Catholic Parish Administrator (approx 35 hrs/wk and an estimated 140 hrs/month) tasks and functions, for Los Angeles AFB (LAAFB) and LAAFB housing annexes 20 miles south at Fort MacArthur, Pacific Heights, and Pacific Crest-San Pedro as defined in this PWS. The quantity of this requirement will consist in one 12-month base year plus four 12-month option years with the tentative dates as follows: a) Base Year contract of 12 Months: - Period of Performance (PoP): 24 May 2013 - 23 May 2014 b) Option Exercise One (1) of 12 months: - Period of Performance (PoP): 24 May 2014 - 23 May 2015 c) Option Exercise Two (2) of 12 months: - Period of Performance (PoP): 24 May 2015 - 23 May 2016 d) Option Exercise Three (3) of 12 months: - Period of Performance (PoP): 24 May 2016 - 23 May 2017 e) Option Exercise Four (4) of 12 months: - Period of Performance (PoP): 24 May 2017 - 23 May 2018 2. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.
