- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.603, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is 00HCCA16-2013-58003 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information: CLIN 0001: Total Price Phoenix Control Ventilaton System Changes in Lab 3417B $ (All work shall be performed in accordance with the Statement of Work (SOW) below.) Your quote shall include the following: • Total Price for CLIN 0001 • Capabilities in accordance with the Statement of Work and Evaluation Factors This acquisition is set aside 100% for small business. This acquisition is under North American Industry Classification System code 238220. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-66. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the System for Award Management (SAM). To register apply via the Internet at http://www.sam.gov. The quote format is at the discretion of the offeror. Include FAR 52.212-3, Offer Representation and Certification with Offer or indicate it is available at the SAM.gov website. STATEMENT OF WORK Phoenix Control Ventilation System Changes in Lab 3417B MORGANTOWN, WV NIOSH FACILITY Scope of work The contractor will provide all engineering, ventilation control system drawings, equipment start-up, on-site calibration & Phoenix controls system material needed. Please also see additional equipment which may be required listed below. Ductwork demolition, installation of new control valves and electrical work related to changes are not included in this statement of work and will be provided by in-house personnel. This project is located in Lab 3417B located at the Morgantown, West Virginia NIOSH facility Tasks to be performed Lab 3417B The vendor will provide air flow engineering and Phoenix Celeris 2 air flow equipment needed to convert this lab to the Celeris 2 System and may require the following equipment. All energy saving features must be incorporated into the engineering of this room. The facility laboratory division has required three gross lab workstations in Laboratory 3417B. Each one will require a 500 cfm exhaust air flow. In order to properly install these stations the following Phoenix control material may be required from the vendor. This equipment and flow rates must be engineered and this list must be verified. The following list is for reference only: Three (3) Phoenix Celeris 2 exhaust valves, 1 per gross lab station. One (1) Phoenix Celeris 2 exhaust valve for the existing laboratory hood. One (1) Phoenix Celeris 2 general exhaust valve for room exhaust. One (1) Phoenix Celeris 2 make up air valve for the room supply air. Additional materials and parts associated with installation and operation of these valves are also required. Project/ Site Conditions A. All work can be performed during normal business hours or after hours. B. Precautions shall be taken so as not to damage adjacent work or structures. C. Products containing post-consumer material (recycled content) Should be selected for use if available. SECURITY REQUIREMENTS Contractor's personnel must complete a security clearance to obtain contractor badged and un escorted access to the work area.. To perform the work specified herein, contractor personnel will require limited access to HHS-controlled facilities and will not require access to sensitive data or access to HHS information systems. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. The Contractor shall notify the Contracting Officer in advance when any new personnel, who are subject to a background check/investigation, will work under the contract and if they have previously been the subject of national agency checks or background investigations. (d) Investigations are expensive and may delay performance, regardless of the outcome of the investigation. Delays associated with rejections and consequent re-investigations may not be excusable in accordance with the FAR clause, Excusable Delays--see FAR 52.249-14. Accordingly, the Contractor shall ensure that any additional employees whose names it submits for work under this contract have a reasonable chance for approval. (e) Multiple investigations for the same position may, at the Contracting Officer's discretion, justify reduction(s) in the contract price of no more than the cost of the extra investigation(s). (f) Language similar to this Security section shall be included in any subcontracts which require subcontractor personnel to have the same frequency and duration of (1) physical access to an HHS-controlled facility; (2) logical access to an HHS-controlled information system; or (3) access to sensitive HHS data/information, whether in an HHS-controlled information system or in hard copy; access to an information system, access to sensitive data, regular or prolonged access to an HHS-controlled facility. (g) Inquiries on matters that affect contract compliance or terms and conditions should be directed to the Contracting Officer or designee. (h) Within seven (7) calendar days after final acceptance of the work specified herein, the contractor shall return all identification badges in accordance with CDC's Policy titled "In- And Out-Processing of CDC FTEs, PSCs, Contractors, and Other Non-FTEs Policy". Contractor employees who separate from service under the contract prior to final acceptance shall be out processed in accordance with that same policy.
