Attachment Justification and Approval for Brand Name requirement The purpose of this Modification is to change the previously posted Request for Information into a Request for Quotation. THIS IS A BRAND NAME REQUIREMENT, PLEASE SEE ATTACHED J&A. This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number for this procurement is FA4830-13-Q-0011 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-64 (2), Defense Acquisition Circular (DAC) 91-13 and Air Force Acquisition Circular (AFAC) 2012-1107. Acquisition set aside: Brand Name North American Industry Classification System (NAICS): 315990. Size Standard: 500 EMP. Request for Quotation (RFQ) should be submitted and shall contain the following information: RFQ Number, time specified for reciept of offers, name, address, telephone number of offeror, discount terms, and acknowledgement of all solicitation amendments (if applicable). Signed offers must indicate quantity, unit price, and total amount for each item. Offers shall include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers shall also contain all other documentation specified herein. PERIOD OF ACCEPTANCE OF OFFERORS: The offeror agrees to hold the prices in its offer firm for a minimum of 30 calendar days from the date specified for receipt of quotes. (End of clause) -AFFAR 5352.201-9101, Ombudsman (NOV 2012) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).
