Description
The Federal Bureau of Prisons, Transitional Drug Abuse Treatment Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health Services in Grand Rapids, Michigan. Location of services should be within a 5 miles radius of the U.S. Post Office located at 1451 Lake Drive, SE, Grand Rapids, Michigan 49506. Performance periods under this contract will be for a one-year base period to begin on October 01, 2013 with four (4) one-year option renewal periods with service ending (if all options are renewed) on September 30, 2018. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. The Following services are required and may be sub-contracted to another licensed contractor. Base Year: Estimated number of Inmates = 55 Substance Abuse Treatment Services: Intake Assessments & Report (one per authorized inmate) for 35 inmates = 35 assessments Individual Counseling Sessions (30 minutes = 1 unit) 48 units (per inmate) x 35 = 1680 units Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 35 = 2520 units Family Counseling (30 minutes = 1 unit) 8 units (per inmate) x 4 inmates = 32 units Individual Transportation (One Way) 96 trips for 55 inmates= 5280 trips Mental Health Treatment Services: Mental Health Intake Assessment & Report (one per inmate) for 20 inmates = 20 units Mental Health Individual Counseling (30 minutes =1 unit) 48 units per inmate for 20 inmates = 960 units Mental Health Group Counseling (30 minutes = 1 unit) 72 units per (TDAT) most recent Statement of Work dated April 2011- No Masters. The request for quote will be available on or after May 29, 2013 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the request for quote will not be available. The site provides downloading instructions. All future information about this acquisition, including quotation amendments, will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about June 28, 2013. Faith-Based and Community -Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. This requirement is unrestricted. The BOP anticipates awarding a firm-fixed price contract. All responsible business concerns may submit a quote which will be considered by this agency.
