Questions and Answers: 1. Shipping & Delivery: The RFQ indicates, that the quote mark Vendor must provide Freight to single location in Montana. Inside Delivery and Installation must be provided to end user locations within the State of Montana quote mark. Please clarify that all units need to be delivered and installed at a single location and please provide the delivery address with zip code for the same. Equipment will be installed at the following locations: 1. Lewistown, MT: Elliptical Trainer 2. Butte, MT: New Elliptical, New Treadmill, 5-100lbs Dumbbell set, and Recumbent Bike 3. Fort Harrison, MT Helena Armed Forces Reserve Center: 5 Treadmills 4. Fort Harrison, MT quote mark Fit To Win quote mark Center: 5 Treadmills 5. Belgrade, MT: Replace 3 Treadmills Offeror may ship to a single location or may have multiple shipments to the installation locations. 2. Installation Location: Please clarify whether the building where the equipment needs to be installed is at the ground level or whether up or down any flights of stairs. All installations will occur on the ground floor. 3. Pricing: The RFQ indicates that we are to quote as: quote mark CLIN 1:Fitness Equipment LUMP SUM (LS) quote mark. Please clarify that you actually want only 1 Lump sum price covering product pricing, freight and installation charges for all the requested items indicated on the RFQ. In other words, you do not require itemized/line-based pricing. Award will be made in a lump sum amount inclusive of equipment, installation, shipping and warranty. Please break these charges out with offer so that price analysis can be completed This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.. This request is a hundred percent set aside for service disabled veteran owned small business (SDVOSB) concerns. The NAICS code is 451110 and size standard is 500 employees. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26, Effective 1 June 2008 & Class Deviation 2005-00001. Statement of Work The Montana Army National Guard requires the following items, Brand Name or Equal: Two (2) CLSX Life Fitness Integrity Cross Trainer One (1) CLSR Life Fitness Recumbent Lifecycle Bike SPECS: Fourteen (14) CLST Life Fitness Integrity Treadmills One (1) (Life Fitness) Iron Grip Urethane Dumbbell Set 5-50lbs One (1) (Life Fitness) Iron Grip Urethane Dumbbell Set 55-100lbs 1:Warranty: All equipment must have a minimum of a 3 year full parts and service warranty. 2:Service: All equipment (except dumbbells) must include access to an equipment service tech capable of responding within a 48 hour timeframe. Equipment Techs must be able to service fitness equipment anywhere within the State of Montana. 3:Trade in Value: Vendor must provide an opportunity to trade in equipment purchased on this contract towards future contracts. 4:Shipping and Installation: Vendor must provide Freight to single location in Montana. Inside Delivery and Installation must be provided to end user locations within the State of Montana. 5:SAM Requirement - Company must be registered on System for Award Management (SAM) before an award could be made to them. If company is not registered in SAM, they may do so by going to SAM web site at http://www.sam.gov. 6:An extract of DFARS 252.209-7997 is attached to this RFQ. This must be completed by the offeror and submitted with any offer. 4. QUESTIONS AND ANSWERS Email all questions to MAJ Bryan Baldwin, bryan.j.baldwin@us.army.mil. Questions must be received no later than 4:00 pm MST April 24, 2013. Answers will be provided with questions on a non-attribution basis and will be posted to the Montana Army National Guard Business Opportunities website as an amendment to this solicitation.
