The last date to submit the required information,listed below, to Ms. Katie Fogle at katie.fogle@navy.mil to attend the site visit has been changed from NO LATER THAN 5:00 PM EDT on May 10, 2013 to NO LATER THAN 5:00 PM EDT on May 14, 2013. 1. Memorandum from Company stating who, what, when where & why. Must be on company letterhead; 2. Color Copies of Civilian Population Registry (CPR) cards, Passports, etc (front and back); and 3. A completed Isa Air Base Access Request Form to include Names, CPR#, DOB, Cell#, Company Name, Organization, Date Range and POC (a blank spreadsheet is provided as Attachment 1).
