This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-13-Q-0421 is issued as a (Request for Quotation (RFQ). This requirement is a Total Small Business Set Aside. NAICS: The NAICS is 811219 and the small business size standard is $19 million. Any firm that is not considered a small business under this NAICS code should not submit a response to this notice. To be eligible to receive a government award, the offeror must be currently registered in System for Award Management or may register with SAM at http://www.sam.gov/ PERIOD OF PERFORMANCE - This base Period of Performance is from the date of award, plus four (4) one-year options. Options may be exercised unilaterally by the government. Contract Line Item NumberProposed cost 0001 Award Year Maintenance Contract per Statement of Work $___________ 1001 Option Year 1 Maintenance Contract per Statement of Work $___________ 2001 Option Year 2 Maintenance Contract per Statement of Work $___________ 3001 Option Year 3 Maintenance Contract per Statement of Work $___________ 4001 Option Year 4 Maintenance Contract per Statement of Work $___________ DESCRIPTION/SPECIFICATION/WORK STATEMENT STATEMENT OF WORK PART A - GENERAL INFORMATION A.1 INTRODUCTION - This is a full service Parts and Labor maintenance requirement for the Ventana Benchmark Ultra system to provide 24/7 Customer Support via phone and 48 hour priority response for onsite Field Support to include all repairs, preventive maintenance and parts. A.2 BACKGROUND - The Ventana Benchmark Ultra system SN# 311758 will be found in the laboratory at the VA Medical Center, Huntington, WV. A.3 SCOPE OF WORK - 1. Unlimited on-site service support by a Field Service Engineer (FSE) Monday through Friday 8 a.m. to 5 p.m. local time (excluding recognized holidays). Customer Support Center shall be available 24 hours, seven days a week to report service issues and provide immediate troubleshooting. All requests for repair service will be dispatched with a 48-hour priority response. 2. Costs for all repair and annual preventative maintenance parts are included. Contractor shall not charge shipping costs for repair/PM parts. 3. One annual Preventative Maintenance service will be performed by an FSE. All PM service is dispatched for completion within 30 days of request. 4. Unlimited on-site applications troubleshooting by a Technical Applications Specialist (TAS) is available Monday through Friday 8 a.m. to 5 p.m. local time. 5. Five (5) days of applications support (i.e. instrument move assistance, protocol assistance, on-site operator training) by a TAS is available Monday through Friday 8 a.m. to 5 p.m. local time. 6. Agreement includes the installation of all operational modifications by an FSE during the next scheduled service visit. 7. Contractor/Vendor shall provide the laboratory supervisor with a copy of the work performed with detail information on repairs performed during the job. The contractor/vendor will invoice the VA Medical Center Huntington annually for the subscription of this service. STATEMENT OF WORK PART C - SUPPORTING INFORMATION C.1 PLACE OF PERFORMANCE - VA Medical Center Huntington WV, 1540 Spring Valley Drive, Huntington, WV 25704. C.2 PERIOD OF PERFORMANCE - This base Period of Performance is from the date of award, plus four (4) one-year options. Options may be exercised unilaterally by the government. C.3 SPECIAL CONSIDERATIONS - C.3.1 Work Hours - The requirement is for scheduled service during normal working hours (8:00 am - 4:30 pm, Monday thru Friday) C.3.2 Site Regulations - All work shall be performed in accordance with VA Medical Center Huntington rules and regulations. All contractor/vendor personnel shall display a VA Medical Center Huntington issued identification badge while performing work on any VA Medical Center Huntington property. C.3.3 Working Conditions/Safety - The contractor/vendor is solely responsible for working conditions on the jobsite, including safety of all persons and property during performance of the work, and for compliance with applicable OSHA regulations. Contract Type: The government anticipates awarding a Firm Fixed Price award with four option years. Award will be made to lowest price quote which conforms to the requirements within this solicitation.
