Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Interested persons may identify their interest and capability to respond to the requirement. Any vendor that believes it can provide the stated supplies/equipment can provide supporting documents demonstrating their products capability and specifications. ALL PRODUCTS MUST BE FDA APPROVED and meet the surgeons' and the patients' specific medical needs/requirement This solicitation W91YU0-13-T-0101, 0010338990 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-66 The associated North American Industrial Classification System (NAICS) code for this procurement is 339113(Surgical Appliance and Supplies Manufacturing ), Size standard is 500 employees. This RFQ is being issued as Firm Fixed Price Unrestricted. ALL PRODUCTS MUST BE FDA APPROVED, AND MEET THE SURGEON/PATIENT REQIREMENTS. These items/supplies are for use at Madigan Army Medical Center, Tacoma, WA. The Surgeon has determined that these products will best meet the patients' specific medical needs/requirements. ALL PRODUCTS MUST BE FDA APPROVED and meet the surgeons' and the patients' specific medical needs/requirement CLIN 0001. Implants Bilateral with rechargeable battery, which is determine by the surgeon. EVALUATION OF FACTORS: a.ALL PRODUCTS MUST BE FDA APPROVED, AND MEET THE SURGEON/PATIENT REQIREMENTS. Award will be made based on Technical Acceptable Only, The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-66.
