Description
Vicinity & Site Map Workers Comp Certificate of Verification Schedule of Items - Service and Prices WD 95-0028 09/18/2012 Description and Specifications SYNOPSIS: AG-02RC-S-13-0059 is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotations and the documents incorporate provisions and clauses in effect through Federal Acquisition Circular 2005-66. This procurement is set aside for Small Business. The NAICS code is 115310 and the Small Business Size Standard is $7.0 million average annual receipts of a firm over three years. The St. Joe Ranger District, Idaho Panhandle National Forests, has a requirement for services necessary to perform a maintenance cycle on the Emerald Creek Garnet Area, which includes the furnishing of all labor, equipment, supervision, transportation, operating supplies, and incidental area maintenance in accordance with the terms, specifications, conditions, and provisions of this solicitation. There may be occasional, unforeseen, incidental area maintenance required by the Contractor's equipment on an hourly basis such as fill or smooth ruts in the road while performing the area maintenance, or smoothing areas around restrooms. Also required is an end of the year clean out of the Fine Settling Pond to be accomplished with an Excavator as described in the attached Description and Specifications. PROJECT LOCATION AND DESCRIPTION: The maintenance work to be performed is located at the Emerald Creek Garnet Area. The proposed project is located in 281 Gulch in the Emerald Creek drainage south of Fernwood, Idaho, in Latah County, T42N, R1E, Section 8. The project location is accessible by vehicle from State Highway 3 and Forest Service Road 447 south of Fernwood, Idaho. The Emerald Creek Garnet Area is 0.2 miles up Road 3781 off road 447. A Vicinity Map and Site Map are included with approximate locations depicted. The equipment used to perform this work shall meet safety standards and shall be in good mechanical condition. One Maintenance Cycle includes removing the wooden boxes around the Sifting Area and then replacing them after the fine material is removed; removing the fine material from the Sifting Area and stockpiling it at the Stockpile Area; removing large rocks from the Digging Pile and hauling to the Stockpile Area; reshaping the Digging Pile; removing the discarded gravel spoils from the Gravel Spoils Deposit Area and depositing this material in a separate pile at the Stockpile Area; loading and hauling the previous week's dried course material from the Sediment Drying Area to the designated pile at the Stockpile Area; and removing the coarse sediment from the coarse settling pond and depositing this material on the Sediment Drying Area to dry. Work will begin approximately May 29, 2013, and end the first week of September, 2013. Maintenance cycles will initially be on a bi-weekly cycle (every 2 weeks). Dates for the initial maintenance cycle to begin are either June 5, 2013, or June 12, 2013. The actual date for the initial maintenance cycle to begin will be dependent on weather and snow melt. The Call-When-Needed (CWN) maintenance cycles are dependent on the level of user activity at the Emerald Creek Garnet Area. The CWN maintenance cycles will be determined by the Contracting Officer and will be ordered as needed via form FS-6300-12 Work Order. Payment will be for actual quantity of work. EQUIPMENT RENTAL: This work consists of furnishing and operating equipment for work directed by the Contracting Officer not otherwise provided for under the contract. Make equipment available for inspection and approval before use. Furnish and operate equipment with such auxiliary attachments, oilers, etc., as are usually needed for efficient operation of the equipment. Keep the equipment in good repair and operating condition. Daily written records, such as shift tickets, to record the number of unit hours of operation are required to be signed and submitted with invoices. Do not measure time for moving equipment between project work sites. Do not measure non-operable equipment or equipment dependent upon another piece of non-operable equipment. The working rate specified in the Schedule of Items shall be paid for all hours of actual operation of equipment on authorized work. Only quantities approved by the Contracting Officer shall be accepted under this contract. The accepted quantities will be paid at the contract price per unit of measurement for the Rental Equipment pay item listed in the schedule of items. Payment will be full compensation for the work done and accepted. The Government reserves the right to reduce or eliminate entirely the work under this item with no adjustment in the contract unit prices. SCHEDULE OF ITEMS: See the attachment entitled Price Sheet. EQUIPMENT SPECIFICATIONS: See the attachment entitled Description and Specifications. MAPS: See the attachment entitled Maps. EVALUATION CRITERIA: The Government will make award to the responsible offeror whose offer conforms to the elements of this Request for Quotation and is most advantageous to the Government. One award will be offered considering price alone. ALL VENDORS MUST HAVE AN ACTIVE REGISTRATION IN THE SYSTEM FOR AWARD MANAGEMENT (SAM) in order to receive an executed Purchase Order from this solicitation.
