- B-702 Demolition and Asbestos Abatement P-400-Contractor to provide all plant labor, equipment, materials, and transportation, necessary to perform Demolition and Asbestos Abatement of B-702, in strict accordance with the 1. Statement of Work (SOW), dated 30 Jan 2013, Project# RVKQ 09-0351, and all attachments (2. ACKNOWLEDGEMENT, AGREEMENT AND CERTIFICATION OF COMPLIANCE WITH CONTRACT INSURANCE REQUIREMENTS CONTRACT NUMBER ____________ 2.1 (a) Site Pictures, 3 ea., 2.1 (b) Map, 2.2 (a) Asbestos Survey Reports by URS Group, Inc. 0106, dated 6 Jan 12 2.2 (b) Asbestos Survey Report by URS Group, Inc. 080511, dated, 5 Aug 2.3 Asbestos Hazard Control Activities 02 82 14.00 10, 29 pages, 2.4 Standard Operating Procedure for Handling of Lamps and Ballasts, dated 1 Nov 11 in 3 pages, 2.5 Refrigerant Accountability Report, 1 page, and 2.6 SECTION 01 57 20.00 10 Environmental Management Protection, dated 15 Jan 11, in 27 pages, 2.7 Wage Determination # 2005-2371, Revision # 14, Dated 13 JUN 13, in 10 pages). This contract is to be accomplished at the 914th Air Reserve Station, Niagara Falls, N.Y. The type of contract that would be awarded will be a Firm Fixed-Price Service Contract. The North American Industry Classification System (NAICS) code is 56170, with a small business size of $6.M. This award is set-aside for Small Woman Owned Business. All responsive responsible sources from Small Women Owned Business concerns may submit a proposal that shall be considered. This requirement consists essentially of, but not limited to the removing and disposing of asbestos containing materials from B-702. Contractor must be properly certified to perform asbestos abatement. Asbestos abatement shall be performed in accordance with all federal and state regulations, including 12 NYCRR 56, 40 CFR 61 Subpart M, and 29 CFR 1926.1101. Contractor is responsible for all aspects of a complete asbestos abatement project including but not limited to notifications, removal, monitoring and disposal. Only Certified Asbestos Abatement removal businesses are to be subcontracted for this contract. Proposals shall be evaluated on Asbestos Abatement procedures adherence to the Statement of Work (SOW) and lowest price, with Price being the highest evaluation factor. Potential bidders should download RFP package FA6670-13-R-0006 from FBO website: https://www.fbo.gov/. No hard copies will be provided. A site visit shall be conducted on Wednesday, 8 May 2013, at 9:00 a.m.. The site visit is not mandatory but it is advised that prospective offerors attend. Site visit attendees shall meet at 9:00 am at B-800 Room 232. Prior to site visit please provide the full name of all individuals attending, DOB, place of birth and drivers license number (if driving). Please submit this to Linda.hatfield.2@us.af.mil 2 days prior to the site visit. NOTE:. Contractors not certified by the State of New York for the purpose of removal, encapsulation, enclosure, demolition and maintenance of structures or coponents covered by or composed of asbestos-containing materials at the time of submission of a proposal, including prices shall be rejected. With Price being the highest evaluation factor. " Notice to Offerors: Funds are not presently available for this project. No award will be made until funds are available to make payments under a contract. The government reserves the right to cancel this solicitation, either before or after bid opening/RFP closing, with no obligation to the offeror by the government."
