THIS IS NOT A REQUEST FOR PROPOSAL (RFP), INVITATION FOR BID (IFB), REQUEST FOR QUOTES (RFQ), REQUEST FOR APPLICATION (RFA) OR AN ANNOUNCEMNT OF A SOLICITATION AND DOES NOT ASSUME THE GOVERNMENT WILL AWARD A CONTRACT OR PROCEED WITH ANY OF THE ABOVE SOLICITATIONS IN THE NEAR FUTURE. THIS NOTICE IS ONLY FOR MARKET RESEARCH PURPOSES. Based on capability statements received from SMALL BUSINESSES ONLY from this announcement, this acquisition may be solicited as a 100% Small Business Set-Aside in accordance with FAR 19.502. The NAICS Code for this procurment is 541511, and the small business size stadard is $25.5 million. All Small Business Concerns are encouraged to respond to this notice. The purpose of this procurement is to develop an integrated surveillance platform to collect HIV, Viral Hepatitis, STD and TB disease surveillance data using web based and non-web based solutions and to include data storage and access services. The development process should have the flexibility for expansion to include surveillance data for diabetes mellitus, Hansen's disease and vaccine preventable and syndromic conditions/presentations. The system must meet all CDC surveillance data validity rules and system requirements. Specifically, the system will have the capability to collect and report all surveillance data variables in the DTBE Report of Verified Case of Tuberculosis (RVCT), data elements now collected in the Division of HIV eHARS system, Division of Viral Hepatitis and Division of STD surveillance reporting forms. In addition, the system will have the ability to collect and report HIV test data (both positives and negatives) currently collected by the USAPI jurisdictions via EpiInfo. CDC will provide access and training on EpiInfo. Finally, the system will also have the capability to expand to include data elements for Hansen's disease, Diabetes and vaccine preventable and syndromic conditions/presentations. The period of performance shall be for one 24- month base period with three 12-month option periods.. Interested organizations should submit their capability statements via email by 2:00 P.M., EDT on May 15, 2013, to the Contracting Office at the following e-mail address scunningham1@cdc.gov. Any questions must be submitted to the Contracting Office in writing to the email address listed. Capability statements will not be accepted after the due date. Page Limit is 25 pages. No facsimile transmissions will be accepted. CAPABILITY STATEMENTS MUST DEMONSTRATE THE MINIMUM REQUIREMENTS OUTLINED IN THE STATEMENT OF OBJECTIVE.
