- Please note: The purpose of this amendment is correct the sole source vendor name for this procurement from Tara Galovski to Amy Williams. This is a notice of intent to award a sole-source contract and is not a request for competitive proposals. The Department of Veterans Affairs Women's Health Sciences Division (WHSD) intends to solicit and enter into a contract with Amy Williams, 600 Parker Square, Suite 210, Flower Mound, TX 75028, for annual Cognitive Processing Therapy services for a national initiative aiming to train VA mental health clinicians nationally in Cognitive Processing Therapy (CPT). The Period of Performance will be from date of award through September 30, 2013. The Government intends to solicit, negotiate and award sole source to Amy Williams, using other than full and open competition procedures pursuant to the authority of the Federal Acquisition Regulations (FAR) 13.106-1(b)(1). The NAICS code for this acquisition is 621330 with a Small Business Size Standard of $7 million dollars. This requirement will be procured under the guidelines of FAR Part 12, Commercial Items. Award will be made applying Simplified Acquisition Procedures, in accordance with FAR Part 13. Amy Williams is the only contractor known who possess the required qualification and training needed to perform these services. "The contractor has demonstrated successful past experience using CPT routinely in clinical settings. "The contractor has demonstrated that she can provide phone consultation to (approximately 8 clinicians at a time). "The contractor has provided a detailed work plan with all tasks and delivery dates clearly indicated. "The contractor has attended the CPT Train-the-Trainer Conference in January 2007 sponsored by the NCPTSD WHSD and the VA Office of Mental Health Services. "The contractor has experience in conducting CPT trainings to groups of 30-40 clinicians at once. "The contractor has been determined eligible to provide these above services by Dr. Patricia Resick, Original Developer of CPT and WHSD Director. However, any contractor capable of providing an equivalent level of service may submit a detailed capabilities statement in order to allow the Government to perform an evaluation of your proposed capabilities/qualifications. This information should be emailed to Darlene Chase, at Darlene.Chase@va.gov no later than May 7th, 2013 at 12PM, Eastern Standard Time (EST). Interested parties shall furnish the following minimum information: 1. Company name and address 2. Company point of contact name, telephone number and email address 3. Evidence that company is a small business, certified SDVOSB or VOSB (or in the process of certification), the number of years in business, affiliate information: parent company, joint venture partners, potential teaming partners. 4. If the company holds a Federal Supply Schedule (FSS) Contract, list the GSA Contract Number and SINs. 5. Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities/qualifications/skills your company possesses to perform services described in the scope of work. Please focus on your company's recommendation for accomplishing the objectives listed above. 6. Past Experience: Brief summary of the company history relative to similar requirements. Offer may provide the following information on a maximum of three similar projects for which the responder was a prime or subcontractor. This notice of intent is not a request for competitive proposals. Capabilities/qualifications statements will be evaluated solely for the purpose of determining whether or not to conduct this procurement on a competitive basis. A determination by the Department of Veterans Affairs not to compete this proposed contract, based upon responses to this notification, is solely within the discretion of the Contracting Officer.
