- Wage Determination Form Past Performance Survey Ambulance Emergency Services in MADISON COUNTY, Huntsville, AL The Bureau of Alcohol, Tobacco, Firearms and Explosives has a requirement to procure Ambulance Emergency Services (AES) located in MADISON COUNTY, Huntsville, Alabama. This is a Solicitation synopsis in accordance with FAR Part 13.5. The method of procurement will be Full and Open Competition. The North American Industry Classification System (NAICS) code is 621910. The Offerors shall have the NAICS code listed in www.sam.gov (formerly www.ccr.gov ) and shall be listed in the Online Representations Certification Application (ORCA). The contract will be a Labor Hour Indefinite Delivery/Indefinite Quantity (IDIQ) Contract. The contract will be awarded by using Lowest Price Technically Acceptable (LPTA) evaluation. Proposals must reference the soliciation number. Complete the pricing section for the base year and four (4) One (1) Option Year Periods. Complete ALL necessary fill-ins and sections. All interested parties must submit a proposal by 5:00 p.m. Eastern Standard time on May 10, 2013. Proposals must include ALL completed sections and requirements.
