Quote Template Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this requirement is F2VVHC3077AC01 and is hereby issued as a Request for Quote (RFQ) using FAR Part 12, Acquisition of Commercial Items. The 6th Contracting Squadron, MacDill AFB, Florida, requires the following item: Renewal of preventative maintenance inspections on 12 pieces of kitchen equipment in accordance with the attached Performance Work Statement (PWS). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-66 effective 01 April 2013 and DFAR DPN 20130419 effective 19 April 2013 and AFFARS AFAC 2013-0327 effective 27 March 2013. The North American Industry Classification System code (NAICS) is 811310, with a Small Business Size Standard of $7.0M. This requirement is being solicited as 100% Small Business set aside. A firm fixed price contract will be awarded. All interested parties must bid on all items. DESCRIPTION OF REQUIREMENT: Base year plus four (4) options years SEE ATTACHED PERFORMANCE WORK STATEMENT AND BID SCHEDULE FOR FULL DETAILS OF SERVICES AND/OR SUPPLIES REQUIRED. BASIS OF AWARD: Award will be made to offer that is Lowest Price Technically Acceptable. The Government will sort all quotes received that conform with the details of the requirement by price, and ensure lowest price is reasonable and realistic. Technical acceptability will be determined when the Government receives certification IAW PWS para. 3.1.3.1. Once a technically acceptable quote has been determined, the evaluation will stop and award will be made. DEPARTMENT OF LABOR WAGE RATE: Wage Determination No: 2005-2125 Revision No: 16 Date of Revision: 13 June 2012 The classes of service employees expected to be employed under the contract: 23110 - Appliance Mechanic SITE VISIT: A one time site visit will be held on Thursday 16 May 2013 at 11:00 am EST. The following clause (FAR 52.219-28) is applicable and shall be completed and submitted with the proposal. 52.219-28 - Post-Award Small Business Program Rerepresentation. (April 2012) (a) Definitions. As used in this clause-- Long-term contract means a contract of more than five years in duration, including options. However, the term does not include contracts that exceed five years in duration because the period of performance has been extended for a cumulative period not to exceed six months under the clause at 52.217-8, Option to Extend Services, or other appropriate authority.
