THIS IS A COMBINED SYNOPSIS/SOLICIATION - NO FURTHER SOLICIATION WILL BE ISSUED This is a combined synopsis/solicitation for commercial item(s) prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation document is issued by the Minneapolis Veterans Health Care System, Network Contracting Office 23, as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Regulations (FAR). This procurement is being issued under Full and Open Competition after Exclusion of Sources, NAICS code 423450. The Contractor/Vendor shall provide the following required item(s): 1.NRG Transseptal Needle, NRG-71-C0, quantity: thirty (30) 2.BMC Radio Frequency Puncture Generator, RFP-100-115, quantity: one (1) 3.Grounding Pad, PMA-GP-BAY, quantity: thirty (30) 4.Connector Cable, RFP-102, quantity: five (5) 5.Footswitch PMG/RFP, PMA-FS, quantity: one (1) 6.RFP Test Box, RFP-100-TB, quantity: one (1) FOB Destination is requested, delivery timeframe will be 45 days ARO, and delivery location will be to Minneapolis VA Health Care System, One Veterans Drive, Attn: Warehouse, Minneapolis, MN 55417 The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. 52.212-2, Evaluation -- Commercial Items applies. Quotes will be evaluated on lowest price technically acceptable (LPTA). Offerors must include a completed copy of the provision at, 52.204-8, Annual Representations and Certifications and submit with its offer. Contractors/Vendors are required to be fully registered and active in the System for Award Management (SAM) at www.sam.gov prior to submitting quote response - this includes being fully updated in the Representations and Certifications area of SAM. Award will be made to the Contractor/Vendor with the lowest price quote response and also meets all the above requirements. VA payment of this awarded purchase shall be made via signed VA purchase order. Offerors shall submit quotes via e-mail only to the VA Contact no later than Wednesday, May 15, 2013 at 12:00 pm CST.
