- Amendment #3 - Adds Equipment and Experience form May 7, 2013 Amendment #3 - This amendment corrects the omission of the Experience and Equipment form (Appendix #4) that is referenced in Section L.9 of the solicitation package. This form was inadvertenly left out of the solicitation package. The form has been attached to this amendment, however the use of this form is optional. Offerors will be required to submit a signed/dated copy of this amendment with any proposal submitted.
