Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The Department of Veterans Affairs Eastern Colorado System at Sheridan Veterans Affairs Medical Center (SVAMC), located in Sheridan, Wyoming shall be contemplating a contract for Optometry Services (Licensed Board Certified Optometrist) on-site at the SVAMC facility. This procurement shall be competed as a Full and Open after Exclusion of Sources, in which, the Network Contracting Office (NCO) 19 is conducting market research to determine if there is a sufficient number of qualified (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business, the other socioeconomic concerns. The Government will use responses to this notice to make the appropriate acquisition decision. The proposed solicitation will be issued in accordance with FAR Parts 12 and 15. The contractor shall provide all aspects of optometry services such as determination of refractive status; determination of binocular vision status; ocular and adnexal health evaluation; external anterior segment and fundus photography with interpretation; epilation of lashes; expression of adnexal glands; examination, testing, dilation, and irrigation of the lacrimal apparatus; removal of non-perforating foreign bodies from the corneal conjunctiva and adnexal; insertion of temporary or permanent punctual plugs; exposed suture removal from the eye or adnexal, after normal healing process, corneal scraping for culturing; uncomplicated repair of non-perforating conjunctival lacerations; determination of visual orientation post Traumatic Brain Injury (TBI). Also, the contractor shall perform refractions, screenings, and optical coherence tomography (OCT) evaluations. The contractor will also provide eyeglass repairs, adjustments, and fittings. The SVAMC will provide the necessary equipment needed to perform the routine contract services. The contractor shall provide OCT evaluation at their private office if the need arises. The total contract period shall be one base year with one year option periods not to exceed four additional years. The total estimated contemplated contract amount shall not exceed $540,000.00. The Selection Criteria shall be in descending order importance: 1) Contract Implementation/Qualifications: Sub Factors: 2) Past Performance ; 3) Quality Assurance; 4) Regulatory Practices including Compliance with HIPPA/OSHA & Company Policies & Procedures; 5) Price. The optometry services schedule shall be performed at the SVAMC as follows: The clinic days at the SVAMC shall be scheduled each week to keep waiting times less than 30 days, in which, the SVAMC will continue to schedule optometry appointments if required. Clinic days shall be determined and provided to the SVAMC at least 45 days in advance. The contractor shall provide an optician or equivalent assistance for all scheduled clinic days. The contractor shall be on call 24 hours/day, 365 days a year to see emergency patients in response to SVAMC as required. The contactor may start their clinic later than 0730 and stay past 1600 with prior approval from the contracting officer. All interested vendors who can meet the requirements stated above should respond, in writing, with information describing their interest, capabilities, geographic location and indicate which small business category for which they qualify to the Contracting Officer shown above no later than 4:00 p.m. (Mountain Time), Friday, May 31, 2013. AGAIN, THIS IS NOT A REQUEST FOR PROPOSAL. The Product Service Code is Q520 Medical Service/Podiatry; the NAICS Code is 621320 and the Size Standard $7.0 Mil. Interested contractors are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System Award Management (SAM) database prior to award of a contract.
