- Notice Information Solicitation: HT9404-13-Q-0020 Agency/Office: Uniformed Services University of the Health Sciences Location: Armed Forces Radiobiology Research Institute, Naval Support Activity, Bethesda, MD Title: Remove and Replace Cobalt-Source 1) Overview: This Request for Quotation was previously referenced under Request for Information number HT9404- 13-R-0020. We are soliciting this requirement using simplified acquisition procedures in accordance with FAR Subpart 13.5. The Uniformed Services University of the Health Sciences (USUHS), located at Naval Support Activity (NSA) Bethesda, MD, has a requirement to support the Armed Forces Radiobiology Research Institute (AFRRI), which is located adjacent to USUHS. A vendor must provide new cobalt sources, for AFRRI's Co-60 Facility, and remove/dispose the existing sources of decaying Co-60. The vendor will be required to disassemble, and reassemble the surface patio which covers the Facility. The Government will make one firm f ixed-price award for the entire requirement. The Cobalt-60 facility is located in the basement ofthe Armed Forces (AFRRI), building. The building is also located on the NSA Bethesda campus. The vendor will· be responsible for disassembling and restoring the outdoor patio which covers the reactor. 2) Statement of Work a. Design and Build: 1. Provide new Co-60 source cassette assemblies (not to exceed 350,000 curies) to produce the following dose rate distributions: (using the existing bi-lateral source elevators) Field size 80 em high x 100 em wide Distance between source assemblies (em) Dose Rate (cGy/min) 400 10, 40, 100 250 250, 1000 100 1500, 5000 Field size 50 em high x 50 em wide Distance between source assemblies (em) Dose Rate (cGy/min) 50 4500, 25000 (or max achievable) 2. Vendor will provide three dummy source assemblies. 3. Source cassettes must match existing configuration. Drawings are available upon request. b. Install: 1. Procure Co-60 source elements (350,000 curies) compatible with source cassettes. 2. Surface contamination on outer cladding <0.005 micro Ci/source. 3. Vendor will transport, ship, and place source assemblies into existing source pool. 4. Government will perform surface contamination acceptance testing of the source transport container prior to refueling. 5. Vendor will provide all handling tools and associated drawings necessary to perform all cassette and source element loading and unloading evolutions. 6. Vendor will decontaminate pool/facility to <0.005 micro Ci. 7. Ensure all activities comply with all applicable regulations and license conditions. 8. Cobalt Facility roof access port will be disassembled and restored to weather tight configuration before completion of the refueling evolution. 9. At no time during the refueling process with the transfer cask be allowed to contract the pool bottom. 10. Vendor will provide all storage racks necessary to store all cassettes, cylindrical arrays, and individual elements. 11. Vendor will train AFRRI personnel on the use of all equipment provided. c. Validate: 1. Vendor will provide all dosimetry required to characterize the facility prior to use. 2. Government will provide acceptance testing to verify the required dose rate distributions. d. Source Removal 1. Remove and take possession of 32 existing Co-60 sealed sources. Information regarding these sources provided upon request. 2. Vendor will remove, transport, and ship the sources from the Cobalt Irradiator pool. 3. It is the responsibility of the vendor to ensure that all activities associated with this contract comply with all applicable regulations and license conditions. 4. Cobalt Facility roof access port will be disassembled and restored to weather tight configuration before completion of the source removal evolution. At no time during the source removal process will the transfer cask be allowed to contact the pool bottom. All requested services must comply with all applicable Federal regulations (to include the Nuclear Regulatory Commission (NRC), and U.S. Department of Transportation (U.S. DoT)), as well as all state, county, and local regulations. All work must be completed and approved by AFRRI no later than 365 days after award. Any delays to this contract will have an adverse effect on the research begin conducted by the Institute. The Contractor must coordinate the installation and remove of Cobalt in advance so AFRRI's impact will be minimized. 3) Government Furnished Material The Government shall provide water and electricity as needed for the installation, validation, and removal of the Cobalt sources. The Government will provide drawings of our existing source cassettes, and technical information on our decaying Co-60. 4) Mandatory Briefing and Escorting Under the Internal Security Act of 1950 and DoD 5200.2-R, Personnel Security Program, AFRRI is a secured building. The Contractor should be aware that employees performing duties under the terms of this contract must have a security clearance prior to the commencing of any work in accordance with the requirements: a. A National Agency Check is required to enter the building without escort. b. A radiation safety briefing will be mandatory for all contractor personnel prior to the start of work c. All contractor personnel must wear dosimeters (a radiation exposure detection device), at all times such personnel are in the radiation source area d. All contractor personnel must have United States citizenship This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. This is a Request for Quotes (RFQ) for commercial items in accordance with the procedures of FAR Part 12 Acquisition of Commercial Items, and Part 13 Simplified Acquisition Procedures. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular (FAC), 2005-66, effective February 28, 2013, and Defense Federal Acquisition Regulation Supplement (DFARS), publication notice 201302228. This requirement is for full-and-open competition under North American Industry Classification System (NAICS) Code 334517- Irradiation Apparatus Manufacturing with a size standard of 500 employees. In order to be eligible for award, offerors must be registered on SAM.gov with completed Representations and Certifications (Reps & Certs). Offerors not registered on SAM.gov with completed Reps & Certs will not be eligible for award. 5) Evaluation Criteria: The government intends to award a single FFP award to the offeror representing the best value to the Government based on the evaluation factors listed below. Evaluation factors are listed in order of importance with most important listed first. The Government will conduct a trade-off between price and non-price factors. The Government is not required to award to the lowest-priced offeror or the highest technically-rated offeror. Evaluation Factors (in order of importance: 1. Technical Capability and Technical Approach: The contractor must provide sufficient literature to demonstrate their capability to handle our entire requirement. The contractor must provide a detailed technical plan, which if executed, will satisfy our entire requirement. The plan must show an in-depth knowledge of our entire requirement, consider the risk factors involved in our requirement, methods to mitigate risk and reduce costs, and provide a milestone plan to complete the project. (The entire requirement must be completed within 365 calendar days of award.) 2. Past Performance on Similar Requirements: In the case of a contractor without a record of relevant past performance or for whom information on past performance is not available; the contractor may not be evaluated favorably or unfavorably on past performance. The Government may use past performance information obtained from other than the sources identified by the contractor and the information obtained will be used for both the responsibility determination and the best value decision. Contractors with multiple representations will be considered more advantageous to the Government. 3. Price: The price offered will be evaluated for the entire requirement. To assist in determining reasonableness, evaluation of a contractor's offer may require pricing from recent contracts for the same or similar requirements. See attached list for applicable provisions and clauses for this requirement. The Government reserves the right to award without discussions. 6) Site Visit, Questions/Comments_;_ The only comprehensive site visit held on MAY 16, 2013. The visit will start promptly at 9:00am in the lobby of Building 70 (A) at the Uniformed Services University, Bethesda, MD. Attendees must allow adequate time to obtain access to the Government facility as Naval Support Activity (NSA) Bethesda (our landlord), requires everyone to have a Government issued ID card, military ID card, or ID card issued by a Federal government agency/activity. Those who are not employed by the federal government, and as a result do not have official government identification, will be required to be placed on NSA Bethesda's access list. In order to be placed on the list, you must provide the following information to Mr. Steve Beck no later than May 10, 2013, 9am Eastern Time: name of the attendee, company name, social security number, date of birth, country of birth, and phone number. On the day of the visit, you must be in possession of a valid driver's license and/or U.S. passport. If you are interested in submitting an offer, but will not be attending the site survey, please send an e-mail confirming your non-attendance. · Questions/comments regarding this solicitation must be received via email to steven.beck@usuhs.mil by May 20, 2013, 6am Eastern Time. Any questions received after that time will not be answered. Questions will not be answered over the phone, nor accepted if submitted by any other means. All questions will be answered in an amendment to the solicitation posted on or about May 27, 2013. 7) Quote packages are due by !O:OOAM eastern time, June 4, 2013. Quotes must be submitted via email to steven.beck@usuhs.mil. Quotes will not be accepted by any other means. Late quotes will not be considered. Quote packages should include a cover sheet that provides the following information: a. Official Company Name b. Point-of-contact, including name, email address, and phone number c. DUNS Number; d. Statement that includes acknowledgement of latest amendment number; and e.
