Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, Streamlined Procedures for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation on a SF1449 will not be issued. The solicitation number is N6264513RCES061. The solicitation is issued as a request for quotation (RFQ). Provisions and clauses in effect through Federal Acquisition Circular 05-66 are incorporated. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses: http://acquisitions.gov/far. The NAICS code is 339112 with a small business size of 500 employees. This is a Full and Open Competition requirement; all qualified vendors are encouraged to submit a quote. The Naval Medical Logistics Command request responses from qualified sources capable of providing a Ophthalmic Examination/Treatment Stands with the following: ESSENTIAL CHARACTERISTICS: OPHTHALMIC EXAMINATION/TREATMENT STANDS Naval Medical Center Portsmouth has a requirement for fifteen (15) ophthalmic examination/treatment stands. Each stand shall be capable of supporting ophthalmic equipment including but not limited to a slit lamp, keratometer, phoropter, examination lamp, and indirect ophthalmoscope. The stand shall have integrated electrical power connection points for supporting examination instruments. The stand shall include wells for housing rechargeable handheld ophthalmic instruments while they are recharging. The stand shall include drawers specifically designed for trial lenses, tissues and other miscellaneous examination aids. The units shall have a control panel to control the installed base of electronically operated Reliance FX920 examination chairs and the currently installed room lighting which includes dimmable stand mounted and ceiling mounted incandescent lights as well as fluorescent lights. The units shall be capable of automatically adjusting room lighting to user-customized conditions upon removal of ophthalmic examination instruments from their storage locations. The treatment stands shall have a footprint of no more than 108 in. x 28 in. The treatment stands shall have a countertop space of at least 2,000 in2. Stands shall not include sinks. The power requirement is 120VAC, 60Hz. Vendor shall be an Original Equipment Manufacturer (OEM) authorized maintenance contractor for the proposed equipment/systems such that OEM warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. The system shall be compliant with the Food and Drug Administration (FDA) requirements to market and deliver medical products for use in the United States of America, even should delivery be requested outside of the United States. The claims made for the product shall comply with the regulations of the FDA with respect to products for marketing and delivery of a medical product for use in the United States of America, even should delivery be requested outside of the United States. The system shall be installed in compliance with OSHA requirements. Upon delivery, the vendor shall be responsible for uncrating the units, transporting them through the facility to the location of intended use for installation, and removing of all trash created in this process. If interim storage is required, the vendor shall make arrangements for the storage. EVALUATION CRITIERA OPHTHALMIC EXAMINATION/TREATMENT STANDS The Government intends to award a contract resulting from this solicitation to the responsible offeror whose offer is the Best Value, price and other factors considere. The technical and past performance factors combined are more important than price, and are listed in descending order of importance. However, price may become a critical factor in the event that two or more offerors are determined to be essentially equal following the evaluation of the non-price factors factors. Evaluation of price will be based on the offeror's total price for all line items. Technical proposals must include sufficiently detailed information to enable evaluation based on the three (3) factors listed below in order of decreasing priority: 1. Technical A.Conformance to Essential Characteristics B.. Design Quality and Capability 2. Past Performance 3. Price Technical: Conformance to Essential Characteristics - The government will evaluate the proposed items' conformance to the essential characteristics for purposes described in the solicitation. Design Quality and Capability - The government will evaluate design quality and capability of the proposed equipment to aid physicians in ophthalmic examination and treatment of patients. Past Performance - The government will evaluate the offeror's recent, relevant past performance record as to (1) the functional performance of the proposed or similar items that the offeror has delivered to previous customers, and (2) the quality and timeliness of the technical assistance, hardware and software updates/upgrades that the vendor has supplied to its customers following the initial fielding of those systems. The government reserves the right to consider other past performance information at its disposal, in addition to any information obtained from the references provided above. The government is likely to randomly select references provided for contact and does not commit to contact every reference. Price Offerors will be evaluated for completeness and reasonableness. Offerors shall provide a firm fixed price for all line items, including delivery. Instructions to Offerors The offeror's initial proposal shall contain the offeror's best terms from a price and technical standpoint. The Government intends to evaluate offers and award a contract without discussions, but reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government reserves the right to award to other than the lowest offeror if the Government determines that a price premium is warranted due to technical merit. The Government reserves the right to award to other than the highest rated offeror. The Government reserves the right to make no award as a result of this solicitation. Offerors shall provide sufficient technical documentation (including but not limited to: product literature, specifications) to allow for thorough evaluation of proposal. Additionally, the vendor shall fully explain their ability to meet the requirements from above highlighting how the proposal adheres to the Salient Characteristics. In addition, the proposal should identify the design quality and capabilities of the proposed system to accept, manage, retrieve, and archive patient information and medical data in the emergency department. Offerors shall provide past performance data for all similar contracts for the last 3 years, up to 5 data points, or 5 data points for delivery starting with the most recent contracts for which delivery occurred at least 6 months prior to the closing of this solicitation. The data shall include: Organization/Location, POC/Number, Contract Reference Number, Contract Date, Value of the Contract (rounding to the nearest $5K is acceptable), and "Has the POC agreed to be to be contacted as a reference?" Shipping shall be FOB Destination. Proposal shall specify the Total Price as well as price for all components. Adequate competition is anticipated for this acquisition. The government anticipates award of a Firm Fixed Price contract. Offerors shall submit firm-fixed pricing for all above requirements. The required delivery date is on or before 10 July 2013.
