Exhibit A - CDRLs Attachment 8 - Sample Task Order Attachment 7 Attachment 6 Attachment 5 Attachment 4 Attachment 3 Attachment 2 Attachment 1 TMS RFP This is a Request For Proposals (RFP) for the Temporary Medical Staffing (TMS) program for the Air Force Medical Service (AFMS). The Government's requirement is for a new personal service contract requirement consistent with DFARS 237.104(b)(ii)(A)(1) to fill temporary (less than 1 year) staffing gaps at 63 military treatment facilities (MTFs) with qualified clinical personnel (e.g., physicians and nurses) for the treatment of military health system (MHS) beneficiaries. While some of the required fills will be predictable and will have more than 30 days lead time, in some instances, required fills will be unpredictable and require rapid fill (the goal is within 30 days). Additionally, there may be instances where a quicker fill is needed and vendors may be asked to meet those requirements. The MTFs covered will include all Air Force CONUS, Alaska, Hawaii, and Guam locations. The Government anticipates establishing multiple Indefinite Quantity Indefinite Delivery (IDIQ) contracts to acquire the clinical direct patient care services. The TMS program will be awarded as a 100% Small Business Set Aside, Indefinite Delivery, Indefinite Quantity, Multiple Award Contracts. The NAICS code for this acquisition is 622110 with a small business size standard of $34.5M. The RFP references the NAICS in the following provisions: FAR 52.204-8 and FAR 52.219-28. Estimated Number of Awards: The Air Force intends to award five Multiple Award Contracts. The contracts will have a five-year ordering period. Task orders may be awarded for up to 364 days. Please see the RFP for additional details. System for Award Management: In accordance with FAR 52.204-99, all offerors must be registered in the System for Award Management (SAM) (https://www.sam.gov/) in order to be eligible for award. Registration requires a DUNS number and may take up to three weeks to process. Recommend registering immediately in order to be eligible for timely award. Instructions for registering are available at the SAM website. Proposals are due by 7 June 2013, 1600 (4:00 p.m.) Eastern Daylight Time (EDT), and shall be addressed to the Contracting Point of Contact (POC) as stated in the RFP. RFP QUESTION DUE DATE REVISION/CLARIFICATION: Any questions on the RFP or any of the RFP attachments shall be submitted no later than 1600, Monday, 20 May 2013 to: Ms. Jennifer Ramirez, Contracting Specialist Jennifer.ramirez@wpafb.af.mil (937) 257-2106 NOTE. Intent to Propose: Offerors that anticipate submitting a proposal for the TMS program are requested to submit an e-mail to the Contractual POC. The e-mail shall include the RFP number, the name of the offeror, the offeror's POC, and a statement relative to the offeror's intent to submit a proposal. The referenced e-mail notice should be provided to the Contractual POC no later than 24 May 2013. This notice of intent is intended to be used for planning purposes only, and failure to submit a notice in no way precludes any offeror from submitting a proposal.
