Description
The Department of Veterans Affairs is seeking interested sources for a requirement entitled: Surgical Laser Services and Instrumentation. The result of this market research will contribute to determining the method of procurement. THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for proposals; submission of any information in response to this market survey is purely voluntary; the government assumes no financial responsibility for any costs incurred. The government will evaluate market information to ascertain the potential market capacity. If your organization has the potential capacity to perform these contract services, please provide the following information: 1) Organization name, address, email address, Web site address, telephone number, and size and type of ownership for the organization; and 2) Tailored capability statements addressing the particulars of this effort, with appropriate documentation supporting claims of organizational and staff capability. If significant subcontracting or teaming is anticipated in order to deliver technical capability, organizations should address the administrative and management structure of such arrangements. Please provide documentation of the size of your business. If you are classified as a small business, Historically Underutilized Business Zone small business, Service Disabled Veteran Owned Small Business, Woman-Owned Small Business and/or SBA 8(a) certified small business, please provide a capability statement in addition to the other information for acquisition strategy determination. BASED ON THE RESPONSES TO THIS SOURCES SOUGHT NOTICE/MARKET RESEARCH, THIS REQUIREMENT MAY BE SET-ASIDE FOR SMALL BUSINESSES OR PROCURED THROUGH FULL AND OPEN COMPETITION. Telephone inquiries will not be accepted or acknowledged, and no feedback or evaluations will be provided to companies regarding their submissions. Submission Instructions: Interested parties who consider themselves qualified to perform the above-listed services are invited to submit a response to this Sources Sought Notice by May 24, 2013 at 4:00 PM MST. The requirement consists of the following Draft Statement of Work: DRAFT STATEMENT OF WORK SURGICAL LASER SERVICES AND INSTRUMENTATION SERVICES REQUIRED: Contractor shall provide qualified surgical laser services and instrumentation for the beneficiaries of the VA Eastern Colorado Health Care System for a base year plus four option years. Services will be provided for the following types of procedures: 1.Otolaryngology 2.Urology 3.Ophthalmology 4.OB/GYN 5.Plastics/Dermatology 6.General Surgery 7.Neurosurgery 8.Vascular Surgery 9.Orthopedic Surgery 10.Oncology 11.Other Procedures as Required 1.1.PLACE OF PERFORMANCE: The VA Eastern Colorado Health Care System is located at 1055 Clermont Street, Denver CO, 80220. 1.2.AUTHORITY: Public Law 104-262 and 38 USC 8153. 1.3.DEFINITIONS/ACRONYMS 2.QUALIFICATIONS: 2.1.Staff/Facility 2.1.1.License: N/A. 2.1.2.Board Certification /Eligibility: Technicians must be board certified and eligible to operate all equipment under all applicable laws and regulations. Proof of certification and continuing education will be provided upon request. 2.1.3.Credentialing and Privileging 2.1.3.1.General Privileges: N/A 2.1.4.Technical Proficiency: Technicians must have documented training to run all equipment for procedures listed in paragraph 1. 2.1.5.The Vendor shall provide all equipment including disposables related to the technician's utilization of laser(s) during surgical procedures. 2.1.6.Contracted services must be available 24/7. 2.1.7.Continuing Medical Education (CME)/ Certified Education Unit (CEU) Requirements: N/A. 2.1.8.Training (ACLS/BLS/VA MANDATORY): N/A. 2.1.9.Standard Personnel Testing (PPD, etc): Contractors will complete all testing as required by VAMC Policy and comply with requests in a timely fashion. 2.1.10.Citizenship related Requirements: Contractors must be a US Citizen and eligible to work in the United States. 2.1.11.Annual Office of Inspector General (OIG) Statement: Contractors must comply with all Office of Inspector General (OIG) investigations as necessary per VAMC policy. 2.1.12.The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel: 2.1.12.1. workers compensation 2.1.12.2.professional liability insurance 2.1.12.3.health examinations 2.1.12.4.income tax withholding, and 2.1.12.5.social security payments. 2.1.12.6.The parties agree that the contractor, its employees, agents and subcontractors shall not be considered VA employees for any purpose. 2.2.TECHNICAL DIRECTION 2.2.1.Contractors will report directly to the Chief, Surgical Services. 2.3.NON PERSONAL SERVICES 2.3.1. No Employee status 2.3.2.TORT 2.4.KEY PERSONNEL AND EMERGENCY SUBSTITUTIONS 3.Hours of Operation: 3.1.Business Hours 3.1.1.Work Schedule 3.1.1.1.Contracted services must be available 24/7. 4.CONTRACTOR RESPONSIBILITIES 4.1.SERVICES REQUIRED: The contractor shall provide the services of a certified laser technician(s) to operate laser(s) for specified surgeries including 4.1.1.Otolaryngology 4.1.2.Urology 4.1.3.Ophthalmology 4.1.4.OB/GYN 4.1.5.Plastics/Dermatology 4.1.6.General Surgery 4.1.7.Neurosurgery 4.1.8.Vascular Surgery 4.1.9.Orthopedic Surgery 4.1.10.Oncology 4.1.11.Other procedures as required 4.2.Vendor Employee Responsibilities 4.2.1.Vendor shall provide upon request the employment information on each referred technician including documentation certifying the technician has completed all the requirements necessary to receive a Certificate of Competence in the field of Laser Technology 4.2.2.Vendor shall provide SOP for laser(s) utilized in the VA ECHCS Operating Rooms and upon request will provide documentation of laser maintenance and safety. 4.2.3.Vendor shall be compliant with the American National Standards Institute (A.N.S.I.) Z136.3 Standard for the "Safe Use of Lasers in Health Care Facilities". Laser operators will work under the authority/guidance of the VA ECHCS Laser Safety Officer (LSO). 4.2.4.Technicians must be able to read, speak and write English fluently and be a United States citizen. 4.2.5.Technician must be able to perform the duties of their assignment under normal and emergent conditions. The vendor will be ready to provide urgent and emergent services within 45 minutes of notification by the VA Medical Center Surgical staff should the need arise. 4.2.6.Worker shall maintain open and professional communication with members of the VA staff, patients and customers. 4.2.7.Vendor shall provide sufficient personnel who meet all the requirements herein and who can be relied upon to perform in strict accordance with the contract. Personnel shall be competent, qualified and adequately trained to operate surgical lasers. 4.2.8.Vendor shall provide a point of contact that will be responsible for the performance of the work. 4.2.9.The VA will conduct criminal background checks on all providers involved in the delivery of surgical laser services. 4.2.10.With consent from the VA, the laser operator(s) may provide surgical services prior to completion of background checks. However, the laser operator(s) shall be within sight and continuous supervision of a VA staff person whose background check has been completed with a favorable result. 4.2.11.All laser operators shall present a professional, conservative and neat appearance. Contractor personnel shall report for duty in a professional manner, in appropriate attire befitting a healthcare setting and have complied with socially acceptable standards of personal hygiene expected of a healthcare worker. 4.2.12.Laser operators shall be current with and have completed all continuing education requirements specified by their professional licensure or certification.
