- Amendment Five Amendment No. Five is hereby issued to Add the following to the Solicitation Scope of Work: 1)Who has jurisdiction over the site: a.The Federal Government has complete jurisdiction over the Federal Reserve on which PIMC is located. However, the authority to regulate Air Quality has been delegated by the Environmental Protection Agency (EPA) to Arizona Department of Environmental Quality (ADEQ) and further delegated by ADEQ to Maricopa County. Therefore, in order to comply with Local, State and Federal regulations, the successful offeror must comply with Maricopa County Air Quality regulations for National Emission Standard of Air Pollutants (NESHAP). 2)What permits are required? a.Refer to General Section B Scope of Work first paragraph: The contractor shall comply with Local, State and Federal regulations concerning disposal of materials. This includes any and all permits required including NESHAP permit. 3)Who will conduct the asbestos survey: a.Refer to Amendment 1- Contractor is responsible for a Hazardous Materials Assessment, if required, prior to disposal. No hazardous Materials are suspected and no Hazardous Material Report is available from the Government.-There has not been an Asbestos Survey conducted by an Asbestos Hazard Emergency Response Act (AHERA) Certified Inspector within the last 12 months. This will be the successful offeror's responsibility. 4)Is a demolition permit required if the trailers are moved off site? a.No, the trailers are already on jacks and therefore do not meet the definition of demolition as defined by Maricopa County regulations. 5)If the survey indicates that no Regulated asbestos-containing material (RACM) is present what permits are required? a.If no RACM is present only a demolition permit will be required to demolish the trailers. If the trailers are not demolished, only removed, no permit is required. 6)If abatement is required who is responsible? a.Refer to General Section B Scope of Work second paragraph The contractor shall remove and dispose of all materials off-site. This includes any hazardous materials, including ACM that are present. Solicitation No. 13-247-SOL-00052 is hereby extended to May 15, 2013 at 12:00pm Phoenix Time. All other terms and conditions of the orginal solicitation remain unchanged. Offeror to sign and submit SF-30 along with other solicitation requirements. Offers must acknowledge receipt of this amendment prior to the hour and date specified in the solicitation or as amended, by completing Items 15(a) through 15(c), and returning One (1) copy of the amendment. Offeror to submit new requirements spelled out in the SF-30.
