Evaluation Criteria Gym Facility in Alexandria, VA REQUEST FOR QUOTE Gym Facility in Alexandria Solicitation Number: HQ0034-13-T-0065 General Information Document Type: Combined Solicitation/Synopsis Responses Due: 29 May 2013 at 10AM EST Set Aside: Total Small Business Contracting Office: Department of Defense, Washington Headquarter Services (WHS) Acquisition Directorate (AD) Requiring Office: Department of Defense (DOD) the Office of Military Commissions (OMC) Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) part 12, "Acquisition of Commercial Items," as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is being issued as a Request for Quote (RFQ). This combined synopsis/solicitation incorporates provisions and clauses are those in effect through Federal Acquisition Circular 2005-66 and DFARS change notice 20130228. Submission of written quotes must be in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Items. Verbal offers will not be accepted. The government intends to award a firm fixed price contract without discussions with respective vendors/quoters. The anticipated period of performance of this contract incorporates a base period (12 months) and two (12-month) option periods. The tentative start date is from July 2013 through June 2014 for the base period and each subsequent one year Option period may commence thereafter as required. The government also reserves the right to conduct discussions if deemed in its best interest. This procurement is being issued on a restricted basis. Only Small Business with The North American Industry Classification System (NAICS) Code is 713940 and the size standard is 7M. This solicitation is a request for quotations (RFQ). The DOD, OMC is seeking a physical fitness facility within a one (1) mile radius located at 200 Stovall Street, Alexandria Virginia 22332. All interested vendors shall provide quotes for the following items. (See evaluation criteria for additional information). Mandatory Registrations: To be eligible to receive an award resulting from this RFQ, the apparent successful quoter must be registered in the System for Award Management (SAM) and registrations must be current. If you're Representations and Certifications are not current, go to https://www.sam.gov/ to register and update your account. There will be no exceptions.
