- FAR Clause 52.212-5 Quote Schedule PDF This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Request for Quotation Number: IHS1197648. The commercial items solicitation is issued as a request for quotations (RFQ), 100% SMALL BUSINESS SET-ASIDE. The RFQ solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-66, effective April 1, 2013. NAICS code 423450 and small business size standard 500 employees. Background: The National Supply Service Center has a requirement for hospital isolation gowns as shown on attached Quote Schedule. Gowns are purchased and stocked by the NSSC and shipped to customers as requested. Occasionally NSSC fills drop ship orders. This is not a brand name request. This is an indefinite delivery, indefinite quantity, firm fixed-price requirement. Quantities listed on the Quote Schedule are estimates only and are derived from the past 12 months purchase history. All items are to be shipped F.O.B. Destination. Deliveries shall be made within 3 to 5 days after contractor's receipt of order. No minimum order requirements. Line item number(s), specifications and quantities - see attached Quote Schedule. Award will be based on best value to the Government: 1. Gowns are made in America. 2. Isolation gowns meet or exceed specification requirements listed on the attached quote schedule. 3. Shipping and delivery meet or exceed requirements listed on attached quote schedule. 4. Past performance. 5. Price. Quotes must include: (1) documentation/product specifications; (2) a sample gown for each quoted schedule line item in order to verify products quoted meet the salient requirements for the corresponding product's form, fit and function; (3) completed quote schedule, delivery terms and conditions, (4) DUNS and EIN numbers (5) quote POC, (6) FSS contract information if applicable, (7) completed copy of FAR 52.212-3 or statement indicating valid SAM registration and (8) 3 references from recent, similar contracts for past performance verification-include contract and contact information. Shipping and Delivery: All orders will be placed by the NSSC and delivered to: (1) NSSC, Oklahoma City, OK, (2) drop ship customer Rosebud PHS Indian Hospital, Rosebud, SD. Shipping: FOB Destination. Delivery shall be made within 3 to 5 days after receipt of order. There must be no minimum order requirements. Date(s) and place(s) of delivery and acceptance and FOB point: Delivery date is 3 to 5 days after receipt of order. Acceptance will be verified at destination. FAR provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. No addenda to the provision. FAR 52.212-2, Evaluation-Commercial Items, applies to this acquisition. Award will be made to the quoter with the lowest priced technically acceptable quotation. The vendor's technical quotation to the Government must demonstrate the Vendor's ability to meet the salient requirements of this solicitation. The evaluation and award procedures in FAR 13.106 apply. Quoters must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its quote or statement indicating valid SAM registration. FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. No addenda. FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition and is attached. Applicable FAR clauses cited in 52.212-5 are checked. All FAR provisions and clauses can be reviewed at: https://www.acquisition.gov/. Additional contract requirement(s) or terms and conditions: 1. No contract financing arrangements. 2. This requirement is a 100% small business set-aside. The Defense Priorities and Allocations System (DPAS) and assigned rating, is not applicable to this requirement. Date, time and place quotes are due: June 5, 2013, 4:00 p.m. Central Standard Time.
