Description

Contractor shall provide all labor, materials, transportation and supervision necessary to provide Yoga Therapy Services at the West Haven and Newington VAMCs. This project is subject to the Service Contract Act.. This solicitation is 100% set aside for small business. The applicable NAICS code is 611620 and the size standard is $13.0 million. Award will be based on the best value to the Government. Evaluation factors include price and past performance/similar experience. The solicitation package will be available on or about May 6, 2013. The date for receipt of quotations is scheduled for May 13, 2013 at 12:00 noon. All relevant solicitation documents may be accessed for download on the date of issue at the following website: http://www.fbo.gov. Any amendments to the solicitation will be posted solely to this web site. It is the responsibility of the offeror to verify the number of amendments issued. Seeking Award of Firm Fixed Contract with a Base Period of 1 Year and a option for 1 additional Year REQUIREMENTS AND DELIVERABLES: The Contractor shall be required to provide the following deliverables: 1. Year 1: Two (2) 12-week sessions of Mindfulness-based Yoga Therapy within a 12-month period to begin May 1, 2013. Each session will be offered twice a week. The program must incorporate 5 practices: Movement, Breathing, Yoga Nidra, Meditation and Gratitude. Sessions will be provided at: 555 Willard Avenue, Newington, CT 06111. Maximum of 20 participants allowed per session Each participant must be provided with a Yoga Practice Guide, Yoga mat, instructional cd/dvd or other materials. Must provide yoga therapy services for a beginner's level participant. Each session will be 75min. 2. Year 2: Two (2) 12-week sessions of Mindfulness-based Yoga Therapy within a 12-month period to begin May 1, 2014. Each session will be offered twice a week. The program must incorporate 5 practices: Movement, Breathing, Yoga Nidra, Meditation and Gratitude. Sessions will be provided between two VACHS campuses: 555 Willard Avenue, Newington, CT 06111. Yoga therapy must be at a beginner's level. Maximum of 20 participants allowed per session Each participant must be provided with a Yoga Practice Guide, Yoga mat, instructional cd/dvd or other materials. Must provide yoga therapy services for a beginner's level participant. Each session will be 75min. 3. Year 2: Two (2) 12-week sessions of Advanced Mindfulness-based Yoga Therapy with focus on resiliency within a 12-month period to begin May 1, 2014. Each session will be offered twice a week. The program must incorporate 5 practices: Movement, Breathing, Yoga Nidra, Meditation and Gratitude. Sessions will be provided at: 555 Willard Avenue, Newington, CT 06111. Maximum of 20 participants allowed per session Must provide yoga therapy services for an advanced level participant. Participants eligible to attend the advanced level must be one of the graduates of the beginner's level sessions or otherwise qualified for the advanced training as determined by the yoga teacher. Each session will be 75min. 4. Year 1: 50-sessions of Mindfulness-based Yoga Therapy provided on twice a week basis located at 950 Campbell, West Haven, CT 06516. Therapist must have experience with providing yoga therapy services to patients with PTSD and mindfulness and provide proof of such experience. The program must incorporate the 5 practices: Movement, Breathing, Yoga Nidra, Meditation and Gratitude. Maximum of 12 participants allowed per session Each participant must be provided with a Yoga Practice Guide, Yoga mat, instructional cd/dvd or other materials. Must provide yoga therapy services for a beginner's level participant. Each session will be 75min. 5. Year 1-2: 8-sessions of Mindfulness-based Yoga Therapy specific to Women Veterans located at 950 Campbell, West Haven, CT 06516. The program must incorporate the 5 practices: Movement, Breathing, Yoga Nidra, Meditation and Gratitude. Therapists must have experience working with Women Veterans. Maximum of 12 participants allowed per session Each participant must be provided with a Yoga Practice Guide, Yoga mat, instructional cd/dvd or other materials. Must provide yoga therapy services for a beginner's level participant. Each session will be 75min. 6. Contractor (or designated therapist) must be a certified yoga therapist graduating from 200 hour yoga training program, should have at least 1000 hours and three (3) years of teaching experience since graduating from the 200 hour yoga training program. 7. Contractor (or designated therapist) must have experience working with Veterans patients who may have the following diagnoses: Chronic Pain, Traumatic Brain Injury, Anxiety, Insomnia, Depression, Drug Addictions, or Anger issues. 8. Contractor (or designated therapist) must have experience in working with patients who have Post Traumatic Stress Disorder and must be able to provide proof of such experience. 9. Services will be provided between two (2) locations within VACHS: 555 Willard Avenue, Newington, CT 06111 and 950 Campbell, West Haven, CT 06516. Contractor must be willing to travel between the 2 locations to provide the services. PRIOR TO DELIVERY OF SERVICE: Session dates will be finalized no later than 30 days after contract award date along with locations of services. Session dates may be subject to change.
