- JOFOC Statement of Work (SOW) The National Heart, Lung, and Blood Institute (NHLBI) is seeking a Contract for diagnostic Testing required under Internal Review Board (IRB) - approved clinical protocol. Title: Notice of Intent to Award Sole Source to Quest Diagnostic Nichols Institute, THIS IS A NOTICE OF INTENT, NOT A REQUEST FOR A PROPOSAL. A SOLICITATION DOCUMENT WILL NOT BE ISSUED AND PROPOSALS WILL NOT BE REQUESTED. The National Heart, Lung, and Blood Institute (NHLBI), Office of Acquisitions (OA), on behalf of the NHLBI/ Hematology Branch, Laboratory (HBL), The Division of Intramural Research (DIR) intends to negotiate and award a purchase order on a noncompetitive sole source basis to Quest Diagnostic Nichols Institute, 14225 Newbrook Drive, Chantilly, VA 20153. Statement of Work: See attached Statement of Work for complete details for the required services. The period of performance will consist of a base year of twelve months and two option periods of twelve months each. Contract will begin upon receipt of order. This notice of intent is not a request for completive proposals. This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-66 April 1, 2013. The corresponding NAICS code is 325199 and the small business size standard is 1000 employees. The basis for sole source determination: See attached Justification for Other Than Full and Open Competition: (JOFOC) See attachment This acquisition is conducted under FAR Part 6.302-1(c) (1), only one responsible source and no other supplies or service will satisfy agency requirements. Interested parties may identify their interest and capability to respond to the requirement, in writing, no later than the response date. Written responses to this notice shall contain sufficient documentation to establish a bona fide capability to fulfill the requirement. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
