- W-9 Form Vendor Questionnaire Form Scope of Work ID #GL 0003 To complete Electrical Type Items listed in the Facility Survey Report that include the following deficiencies: #50316, #91813, #91814, #91796, #91797, and #91803 Table of Contents I. Proposal Information Project: To complete Electrical Type Deficiencies items at the following Locations: Site, Auto Mechanic/Facilities Maintenance Training Building #8, Education Building #5 and Gymnasium & Recreation Building #7. At the Glenmont Job Corps Center Owner: US Department of Labor (DOL) Employment and Training Administration Center Operator : Adams and Associates, Inc Mailing address Glenmont JCC, 822 River Road, Glenmont, NY 12077 Proposal Due Date: 5 PM on June 20, 2013 Evaluation Criteria: Will be based on Cost 60% Quality 20% Past Performance 10% Schedule 10% Payment: Payment Terms are Net 30 Days of completion and receipt of Invoice Site Visit Date: June 4, 2013 at 9:30 am. Please contact Greg Guay, Glenmont Purchasing Agent, if you will be in attendance. a) Contractor is mandated to visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted with ID #GL 0003 will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in the performing same. This proposal shall be valid for a period of 90 days. II. Project Description Scope of Project: To complete the following Electrical Type Items.Items include the installation of lights and poles at the ball field, provide heavy duty rolling door operator kit for the auto shop overhead doors, provide one industrial type cord reel for the auto shop where they currently they are using an extension cord. In addition located in the center's auditorium there are two issues that need to be resolved: Light fixtures that do not work need to be replaced and Stage Lighting controls in disrepair that need to be replaced. Three light fixtures located at the gym are in need of replacement.xs Def #50316 - (recreation field cannot be used in the evenings) Def #91813 - (Shop Overhead doors are difficult to operate) Def #91814- (inadequate electrical connection for the alignment lift) Def #91796 - (Light fixtures in auditorium not operational) Def # 91797- (stage lighting control systems in disrepair) Def # 91803- (three exterior light fixtures are deteriorated and not operational) III Specific Requirements: Install (12) 30' high poles with (20) pole- mounted light fixtures around the baseball field located northwest of building #8. At building #8 Auto Shop overhead doors need to be replaced. Also in the Auto Shop install an electrical connection for the alignment lift. In the center's auditorium the light fixtures need to be replaced and the stage lighting control system is in total disrepair and needs to be replaced. Located at the Gymnasium &Recreation Building there are three light fixtures that are not operational and need to be replaced. NOTE: A packet containing the exact location of all 6 project deficiencies will be given out at the pre bid walk thru which explains in detail the work to be completed. Warrantees: •a) The contractor is to provide written warranty for the above statements at time of completion of job IV Construction Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. The center will provide a copy a copy of the centers rules and regulations to the contractor. Management of Jobsite: The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. The Contractor shall clear all deliveries with the Center's security personnel. Wage Rates: The bidders shall conform to the latest Davis-Bacon prevailing wage rate decisions for the area. (See the attached wage rate) ELEC0236-001 06/01/2012 ALBANY; COLUMBIA; FULTON; GREENE (that portion north of the line following the south limits of the city of Catskill); MONTGOMERY; RENSSELAER, SARATOGA, SCHENECTADY; SCHOHARIE; WASHINGTON. Rates Fringes ELECTRICIAN................... $33.50 3%+ 19.02 Insurance: The Contractor shall obtain and maintain insurance as follows: Workman's Compensation/Employers Liability insurance, to meet New York statutory requirements Comprehensive General Liability ($1,000,000 per occurrence) Comprehensive Automobile Liability ($1,000,000/combined single limit) Fire Legal Liability ($500,000 per occurrence) The Contractor must cover any deductible costs. Upon contract award, the Contractor must add Glenmont Job Corps academy as additional named insured to the policies. Schedule: The Contractor shall submit a schedule for the work. The project shall be completed within 90 calendar days of the award of the contract. Written authorization is required to extend the 90 day completion requirements. Final Acceptance The Contractor shall notify the Center's facility manager when the project is substantially complete, and shall request an inspection. The completion of this inspection shall determine the date for the commencement of the warrantees, and the date for payment. V. Proposal Form The Contractor's proposal shall be submitted on company letterhead, must reference the scope ID # and address all items in section II and III, to be considered. The Contractor shall submit the following information with the proposal: A list of materials and product data sheets (MSDS) proposed for use. References Copies of Insurance Certificates All questions must be in writing and sent to the Purchasing Agent Guay.greg@jobcorps.org No Phone Calls will be accepted.
