Description

This Pre-Solicitation Notice is for artificial limbs and related services for the Department of Veterans Affairs, Alexandria VA Health Care System located at 2495 Shreveport Highway, Pineville, Louisiana. The vendors will be required to fabricate all artificial limbs in strict conformance to the prosthetic prescription provided by the VA Amputee Care Clinic, Prosthetics and Sensory Aids Service. Veteran beneficiaries will normally be measured, fitted, and accept delivery of their prosthesis at the contractor's facility. Therefore the vendors receiving award shall have a facility within the Alexandria VA Health Care System catchment area. This catchment area encompasses the major cities and outlying areas of Alexandria, Lafayette, and Lake Charles, Louisiana. This includes the following areas: Parishes of Franklin, Caldwell, Concordia, Catahoula, La Salle, Grant, Winn, Natchitoches, Vernon, Rapides, Avoyelles, Evangeline, St. Landry, Allen, Beauregard, Calcasieu, Cameron, Jefferson Davis, Acadia, Vermilion, Lafayette, St. Martin, and Iberia. The period of performance is anticipated to be July 1, 2013 to June 30, 2014, with four (4) one-year options exercisable at the discretion of the Alexandria VA Health Care System. It is the VA intention to solicit proposals and award multiple contracts (a maximum of 5) on a competitive basis to qualified providers within the Alexandria VA Health Care System's jurisdiction under the authority of Title 38 U.S.C. Section 8123. The NAICS Code for this procurement is 339113; size standard is 500 employees. This procurement will be full and open competition. Your response to the request for proposal must contain all requested pricing and non-pricing information in accordance with the solicitation/contract terms, conditions, and provisions. The VA anticipates issuing a solicitation for this requirement on or about May 24, 2013 with an anticipated response closing date on or about June 14, 2013. This solicitation will be available for download from http://www.fbo.gov. Interested contractors will be responsible for downloading copies of the solicitation and any subsequent amendments from that website. No hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments. All responsible sources may submit proposal which shall be considered by the Agency. To be eligible for award, contractors must be registered in the System for Award Management (SAM) data base at https://www.sam.gov/portal/public/SAM/. Registration is free and potential offerors are encouraged to visit the SAM website and register as soon as possible to avoid delays in the event they are selected for a contract award.
