- SOURCES SOUGHT: Request for Capable Offerors THIS SOURCES SOUGHT IS FOR THE FOLLOWING LOCATIONS: HAMPTON VA MEDICAL CENTER IN HAMPTON, VA AND HUNTER HOLMES MCGUIRE VA MEDICAL CENTER IN RICHMOND, VA. NOTE: THERE ARE 2 PERFORMANCE WORK STATEMENTS - 1 PWS FOR HAMPTON VAMC AND 1 PWS FOR RICHMOND VAMC The contractor shall provide laboratory support including removable prosthodontics laboratory work for VAMC Hampton and Richmond. The contractor shall provide all materials and supplies necessary to complete each item. See the two (2) PWS's included in this RFI. THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE. This Request for Information (RFI) is issued solely for information and planning purposes and does not constitute a solicitation. Responses to this RFI will not be returned. It is the intent of the Contracting Office to use this for market research purposes. The Government shall not reimburse the costs of preparing responses to this request for information. The Department of Veterans Affairs Network Contracting Office is seeking potential small business sources. The Government anticipates issuing a commercial Request for Proposal (RFP) for Removable Dental Services in support of the Hampton VA Medical Center and Hunter Holmes McGuire VA Medical Center. This proposed contract is being considered for a set-aside under a small business set-aside program. The North American Industry Classification Systems (NAICS) Code proposed for the requirement is 339116 - Dental Laboratories and the business size standards is 500 employees. The Government is interested in all small businesses. Please indicate if your company is any one or more of the following including: 8(a), Historically Underutilized Business Zone (HUB Zone), Service Disabled Veteran Owned Small Business (SDVOSB), Veteran-Owned Small Business, Woman-Owned Small Businesses (WOSB), Minority-owned Small Business, or any other socio-economic small business status, that are interested in performing this requirement. The Government requests interested parties submit a brief description of their company's capabilities, business size (i.e. annual revenues and employee size), business status (i.e. 8(a), HUB Zone, WOSB, SDVOSB), anticipated teaming arrangements, and a description of similar services offered to the Government and to commercial customers. Any responses involving teaming agreements should delineate between the work that will be accomplished by the prime and the work accomplished by the teaming partners. The Government will use this information in determining its small business set-aside decision. Respondent must be able to meet specifications required in the Draft Performance Work Statement (s) (attached). CAPABILITIES STATEMENT: Interested parties must submit a Capabilities Statement, with the cover sheet provided completed, brief and concise, yet clearly demonstrating ability to meet the stated requirements. Interested companies may submit a capabilities statement not to exceed 5 typed pages 8.5x11, to include flyers and miscellaneous advertisements about your company. The Capabilities Statement should clearly present evidence that the interested party feels is relevant. Capabilities Statements are to include the information in the attachment to this sources sought synopsis and title the Subject line for the response: Hampton VAMC CAPABILITIES STATEMENT: Your Company name. IDENTIFY THE VA MEDICAL CENTER(S) FOR THE CAPABILITIES STATEMENT BEING SUBMITTED AND SPECIFY IF YOU ARE INTERESTED IN BOTH LOCATIONS.
