Description

This is a combined synopsis/solicitation for commercial service identified below, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation VA-263-13-Q-1885 is being issued as an unrestricted Request for Quotation (RFQ) and incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2005-66. The North American Industry Classification System Code (NAICS) is 325412 with the small business size standard of 750 employees. The Sioux Falls VA Health Care System, Sioux Falls, South Dakota, VISN23, has a requirement to procure Radioisotopes per the following specifications: (1) Licensed Radiopharmacy to provide all radiopharmaceuticals listed in the attached schedule for a Base Year period 7/1/2013 - 6/30/2014 with one Option Year, (2) Orders will be placed one day prior to requirement for those pre-scheduled (90% of patients), (3) For those patients not pre-scheduled (approximately 10% of patients), orders will be phoned-in the same day as testing material is required, (4) Contractor must be able to provide up to three (3) deliveries per day as required. First delivery of day will be accomplished approximately at 6:15 am with other deliveries performed within one hour of call. Costs indicated in Schedule of Items include all delivery costs (no Separate delivery charges). Contractor must also provide the following services: (a) Must furnish VA a copy of its radiopharmaceutical license; (b) Must provide 24-hour round-the-clock coverage; (c) Must accept delivered syringes and vials (used and unused) returned by VA for disposal, and (d) Must provide a copy of proper training documents of shippers and carriers of radioactive materials. This solicitation incorporates the following FAR clauses, provisions and addendums: 52.212-1, Instructions to Offerors- Commercial Items (Feb 2012); 52.212-3, Offeror Representations and Certifications-Commercial Items (April 2012); 52.212-4, Contract Terms and Conditions-Commercial Items (Feb 2012); 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (July 2012) (including the following item numbers in paragraph (b): 1,4,6,8,10,14,23,26,28,29,30,31,32,33,34,38,40,42,48,49 and 52.216-1, 52.216-18, 52.216-19, 52.216-21, 52.216-22, 52.217-8, 52.217-9, 52.232-19; and the following VAAR Clauses and Provisions; 852.203-70 Commercial Advertising; 852.215-70 SDVOSB and VOSB Evaluation Factors; 852.215-71 Evaluation Factor Commitments; 852.216-70 Estimated Quantities, 852.237-7 Indemnification and Medical Liability Insurance; 852.246-70 Guarantee; 852.246-71 Inspection; 852.270-1 Representative of Contracting Officers; 852.273-74 Award Without Exchanges; and 852.273-76 Electronic Invoice. The Government shall award a contract resulting from this solicitation to the responsive/responsible offeror/s whose offer conforming to the solicitation will be most advantageous to the Government per FAR 13.106-2. Award will be made on the basis of the lowest evaluated price of proposal meeting or exceeding the acceptability stands for the following non-cost factors: Technical Capability to meet the 5 specifications listed above and the Past Performance of relevant experience. Provide three (3) references, preferably of Government customers, performed in the last five (5) years. Responses shall include descriptive material and other such information corresponding to each minimum required item, which demonstrates the Offeror's capabilities, and shall identify where the offered service meets or does not meet each of the Governments functional and performance minimum specifications listed herein. Information concerning clauses and provision incorporated by reference may be obtained at http://www.acquisition.gov. Quotation is due by 4:00 pm CT on Monday, June 3, 2013, with anticipated award date of 7/1/2013.
