Description
RFQAG-3198-S-13-0006 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The United States Department of Agriculture (USDA), Food and Nutrition Service (FNS) anticipates entering into a single award contract for the procurement of services to study and report on the Evaluation of the 2013 Summer Food Service Program Participant Characteristics. This solicitation is being released under FAR Part 12, Commercial Items Acquisition, in conjunction with FAR Part 13, Simplified Acquisition Procedures, for proposal evaluation. The NAICS Code for this RFQ is 541720, Research and Development in the Social Sciences and Humanities. This solicitation is issued as a Request for Quote (RFQ) for a 100% Small Business Set-Aside. The Food and Nutrition Service (FNS) of the United States Department of Agriculture (USDA) seeks the services of a qualified Offeror to collect data on a nationally representative sample of States, sponsors, and sites, augmented by qualitative data collection with participants and parents/caregivers to create descriptive data files, analyze this data and prepare a report that will document Summer Food Service Program (SFSP) provider and participant characteristics. In addition, the Offeror will conduct a sub-study of States with high SFSP participation and States with low SFSP participation to examine factors that account for differences in SFSP participation across States. Written questions concerning this RFQ should be submitted to ashley.owens@fns.usda.gov by 4:00 PM Eastern Daylight Time (EDT), May 29, 2013. Please include the solicitation number in the Subject Line for all questions sent via email. Proposals are due on July 8, 2013 no later than 4:00 pm Eastern Daylight Time. Separate electronic copies of the technical and business proposals, in MS-Word and pdf formats, shall be emailed to Ashley.Owens@fns.usda.gov. As separate attachments, cost template spreadsheets shall be included in unprotected (read/write) MS-Excel format.
