Performance Work Statement for Required Repair COMBINED SYNOPSIS SOLICITATION SP4530-12-Q-0002 SHREDDER REPAIR DLA DISPOSITION SERVICES GUAM The DLA Disposition Services/Defense Logistics Agency, Hawaii requests responses from qualified sources in response to this combined synopsis/solicitation. This synopsis/solicitation for a commercial service is prepared in accordance with the prescribed format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. All vendors providing quotes are responsible for monitoring this site for the release of any amendments or other information pertaining to this solicitation. (i) This solicitation is issued as a request for quotation (RFQ). Submit written quotes only. Also see "Instructions to Offerors" and "Evaluation of Quotations" herein. (ii) The RFQ number is SP4530-13-Q-0002. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-66 (Effective 01 Apr 2013) and DFARS DPN 20130419 (Effective 19 Apr 2013). (iii) This notice of intent is a request for unrestricted competitive quotes as this procurement is not set-aside for any small business categories. The North American Industry Classification System (NAICS) Code is 811310 and the Small Business Standard size is $7,000,000. (iv) The FOB Point is Destination which is Naval Base Guam and the quotation shall include separate costs for repair and travel expense which shall include transportation, lodging and per diem. CLIN 0001 Cost to repair SSI Shredder $ _________ CLIN 0002 Travel to/from Guam $ _________ (v) Description of the requirement is as follows: The contractor shall provide all personnel, equipment, tools, materials, supervision, other items, and non-personnel services necessary to perform the repair of the SSI M70H shredder located in facility 632-A at DLA Disposition Services Guam located at Naval Base Guam back to proper working condition as defined in this performance work statement (PWS). Proposers shall review Attachment 1, Performance Work Statement for complete performance requirement. (vi) APPLICABLE CLAUSES AND PROVISIONS. (viii) EVALUATION OF QUOTATIONS/OFFERS: FAR 52.212-2, Evaluation Commercial Items, the following factors shall be used to evaluate offers: (1) Technical capability to perform the Government requirement, (2) Price. This procurement will be evaluated based on low price technically acceptable. Technical acceptability is determined to be the offeror's ability to provide the required services in accordance with the performance work statement and in compliance with the delivery schedule. Award will be made to the responsible offeror whose quote is technically acceptable, in full compliance with all other requirements set forth in this combined synopsis/solicitation, and the lowest evaluated price. The Government reserves the right to judge which quote shows the required capability. The Government also reserves the right to eliminate from further consideration those proposals which are considered nonresponsive or unacceptable. Contractor shall not be eligible for award unless company is actively registered in the System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM. Pursuant to FAR 33.103(d)(4), agency protests may be filed directly with the appropriate Reviewing Authority or a protester may appeal a decision rendered by a Contracting Officer to the Reviewing Authority. (ix) INSTRUCTIONS TO OFFERORS: Questions, inquiries, and quotes must be e-mailed to Bernadette S. del Rosario-Simmons, e-mail bernadette.delRosario-Simmons@dla.mil. Quotes may also be mailed to DLA Disposition Services, Hazardous Acquisition Branch Pearl Harbor J762, Attn: Bernadette del Rosario-Simmons, 1025 Quincy Avenue., Ste. 2000, Pearl Harbor, HI 96860-4512. Contractor is responsible for verifying with contracting officer that quote has been received. Facsimile quotes will not be accepted. All vendors interested in submitting a quote are required to submit the following: quote for items, company name, address, telephone number and point of contact, fax number, e-mail address and commercial and Government Entity (CAGE) code, tax identification number, prompt payment terms, delivery time, date offer expires and warranty information. Quotes shall reference the RFQ number, as the RFQ all terms therein will be referenced in the award documentation. A completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (Aug 2009) shall be completed on the System for Award Management (SAM). The quoter shall use the Standard Form 1449, "Solicitation/Contract Order for Commercial Items." The form can be found at www.gsa.gov. All questions must be received no later than 4:00 PM Hawaii Standard Time (HST) on 31 May 2013. Quotes are to be received no later than 4:00 PM HST on 21 July 2013.
