Description
Performance Work Statement This is a Sources Sought Notice. This is NOT a solicitation for proposals, proposal abstracts or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are small business; HUB Zone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition strategy, and whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. This requirement is assigned a code of 561110 in the North American Industry Classification System (NAICS), and the size standard for such requirements is $7M. General Scope Congress has provided funding annually to the Indian Health Service (IHS) for the Special Diabetes Program for Indians (SDPI) for the prevention and treatment of diabetes. The funds were distributed through non-competitive and competitive grants to IHS, tribal and urban health entities. Of these grants, there were competitive grants (called demonstration projects) in American Indian/Alaska Native (AI/AN) communities which implement activities to prevent diabetes in high risk individuals (Diabetes Prevention or DP sites) and prevent cardiovascular disease in people who already have diabetes (Healthy Heart or HH sites). The IHS originally contracted services to provide technical assistance, development and maintenance of a communication network with the grantees, management and analyzing of data process and outcome measures, which resulted in establishment of The Coordinating Center. The Coordinating Center works under the guidance and leadership of the IHS Division of Diabetes Treatment and Prevention (DDTP) to oversee the day-to-day operations of the SDPI Initiative grant programs and provide technical assistance. It maintains a communications network with the grantees and DDTP, as well as continues to manage and analyze data processes and outcome measures to ensure that it is timely, accurate, and complete. IHS is seeking a Contractor to continue these technical and administrative supports to The Coordinating Center. Please see attached Performance Work Statement (PWS) for more details. Anticipated Period of Performance: The anticipated period of performance for this requirement includes one 12-month base period, with two 12-month options periods. Capability statement/information sought: The response to this Sources Sought Notice must demonstrate the company's technical understanding of the requirements and capability in a concise, logical manner and should not contain superfluous material not directly related to the PWS. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's capability to meet the requirement. Information Submission Instructions: 1. Page Limitations: Interested qualified small business organizations should submit a tailored capability statement for this requirement not to exceed 10 single-sided pages including all attachments (single spaced, 12 point font minimum) that clearly details the firm's ability to perform the aspects of the notice. Tailored capability statements should also include an indication of current certified small business status. This indication should be clearly marked on the first page of your capability statement (preferably placed under the eligible small business concern's name and address) as well as the eligible small business concern's name, point of contact, address and DUNS number. 2. IHS Point of Contact: All capability statements sent in response to this Sources Sought Notice must be submitted electronically (via email) to Reginald Lee, at Reginald.lee@ihs.gov in MS Word or PDF format. The subject line must specify 13-236-SOL-00027. Facsimile responses will not be accepted. This synopsis is for information and planning purposes and is not to be construed as a commitment by the Government, nor will the Government pay for information solicited. Information received will be considered solely for the purpose of determining whether sufficient small businesses possess the experience, expertise and qualifications to ensure competition under a set-aside acquisition. A determination by the Government not to compete this requirement as a set-aside based upon responses to this notice is solely within the discretion of the Government. Please confirm the receipt of the information by the IHS point of contact specified above.
