FBO NOTICE VA259-13-Q-0610 TITLE:Floor Stripping and Re-waxing NAICS:238330 - Flooring contractors NAICS SBA SIZE STANDARD: $14.0 MILLION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA259-13-Q-0610. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-66. The associated North American Industrial Classification System (NAICS) code for this procurement is 238330 with a small business size standard of $14.0 million. This requirement is a total Small Business set-aside and only qualified offerors may submit bids. This is being competed as Lowest Price Technically Acceptable. The Salt Lake City Veterans Affairs Medical Center (VAMC) has a requirement for the stripping of old wax and applying new wax over the surface of approximately 22,501 square feet of tunnel floors in the Salt Lake City VA Medical Center Campus located at 500 Foothill Drive, Salt Lake City, UT 84148. STATEMENT OF WORK The contractor shall provide all supplies, equipment and labor to completely strip and wax 22,501 square feet of tunnels throughout the VA Medical Center campus located at 500 Foothills Drive in Salt Lake City, and adhere to the following requirements: 1) In advance of starting work, the contractor shall provide the government with a work schedule to include which areas will be done each night and how many employees will be assigned to work in each area. 2) The floors are all 12" tile and must be completely stripped down to bare surface and thoroughly cleaned before any new wax is applied. 3) The contractor shall apply a minimum of four (4) coats of a high solids floor wax, allowing sufficient time for each coat to dry completely before starting the next coat. 4) The final product will have a smooth, high gloss finish with no ripples in the finish. 5) The contractor shall ensure that no stripper or wax run under doors into adjacent areas and if that happens, the contractor shall be responsible to correct the problem within 24 hours of the problem being identified. 6) The contractor shall ensure there is a straight clean edge where the wax stops at all doorways. 7) The contractor shall not start work earlier than 9:00 pm each night and shall be done in time to ensure the last coat applied is fully dry before 5:30 am the next morning. 8) The contractor shall ensure any stripper or wax splashed on the baseboards or walls, is properly cleaned off before it dries. 9) The contractor shall place proper warning signs around the area they are working to warn people of the slippery floor danger. 10) The contractor shall stop work and allow emergency passage of medical personnel when needed. 11) The Contractor shall comply with all federal (OSHA), state and local safety policies and regulations to ensure the patients, staff and visitors of the medical center, as well as the contractor's own employees are protected from injury during the process. 12) No contractor equipment shall be left unattended at any time during the term of the contract. 13) The contractor shall take all equipment and supplies with them when they leave work each day. The VA will not be responsible for lost contractor equipment. 14) The Contractor shall only use environmentally safe chemicals, and must provide the COR a Material Safety Data Sheet (MSDS) for all chemical products used on site. The VA reserves the right to approve or disapprove all chemical products used for this contract. 15) The Contractor shall be fully liable for any damage caused to the government facilities. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines.
