FOR FAMILY READINESS COORDINATOR AT SHAPE BELGIUM 1.0 GENERAL: This Performance Work Statement (PWS) outlines the duties and responsibilities of the Family Readiness Coordinator (FRC) who shall provide Family Support Center (FSC) programs and services as set forth herein to eligible military members, retirees, Department of Defense (DoD) civilian members and their families. The FRC role is to assist Commander's in their responsibility to ensure the health and welfare of the military community and support mission readiness by helping eligible personnel and their family members, adapt to the changes and demands of military life. 2.0 CONTRACTOR RESPONSIBILITIES: 2.1 The contractor shall provide all personnel, transportation, labor, and materials to provide a family readiness coordinator. The contractor shall be required to perform, at a minimum, the following functions: 2.1.1 Assess clients' needs and concerns (e.g., family separation, parenting, personal financial management, etc.); evaluate information gathered in interviews and assist or appropriately refer to available helping agencies. Provide guidance and additional referral options to ensure clients acquire their necessary resources. Provide individual and family consultation services, education, referrals, and follow-up. Refer difficult or complex situations to the First Sergeant, Life Skills, and/or Main Operating Base (MOB) FSC. Conduct a variety of educational forums such as individual assistance, classes, seminars, briefings and/or workshops to enhance community capacity and thus assist serviced populations in improving work/life skills and knowledge of available resources. 2.1.2 The contractor shall administer and coordinate Family Support programs and services to meet the needs of the local military community in accordance with AFI 36-3009, Family Support Center Program (found at http://www.e-publishing.af.mil/shared/media/epubs/AFI36-3009.pdf), to include Compliance and Standardization Requirements List (C&SRL). Family Support Center programs consist of Sponsorship; Transition Assistance; Career Focus (spouse employment); Relocation Assistance (including Family Services Loan Locker); Volunteer Resources; Information and Referral; Family Life Education; Readiness; and Personal Financial Management (including Air Force Aid Society emergency assistance referrals). 2.1.3 The contractor shall act as point of contact for distribution of information and publicity flowing to the Det 3 community from the Spangdahlem AB FSC. This includes disseminating information or working closely with the Spangdahlem AB FSC Geographically Separated Unit (GSU) POC to coordinate additional classes, seminars, briefings and/or workshops to be delivered to Det 3 customers by the contractor and/or Spangdahlem AB FSC staff members during periodic visits. 2.1.4 The contractor shall develop and maintain an up-to-date continuity binder on Family Support programs and services available at Det 3. 2.1.5 The contractor shall identify and communicate needed resources and recommend changes to the commander as required, to maintain a viable program that continues to meet the needs of the personnel and family members employed at or living in the Det 3 area. NOTE: The contractor is required to travel to Spangdahlem AB for a five-day orientation training at the Family Service Center within the first 30 days of contract with the Family Support Center program managers, a two-day continuation training will occur within 180 days of award. Both training sessions are at the contractors' expense and reimbursement will not be provided. 2.2 Additional duties the contractor may perform include but are not limited to the following: 2.2.1 Act as the liaison between the 6th ASG Army Community Services "Welcome Everyone to Belgium" volunteer program and the Spangdahlem Family Support Center to maximize programs and services available to the Det 3 community while minimizing duplication of efforts. 2.2.2 Attend the Commander's staff meetings as required and provide slides as requested by the Commander or his Exec that show the health of the program. 2.2.3 Act as liaison between Commander, Spangdahlem FSC and volunteers for the Key Spouse Program. 2.2.4 Develop a Resource Referral Guide (RRG) unique to SHAPE and update it on a quarterly basis. The contractor will utilize Spangdahlem AB RRG as a template for the new guide. The guide is intended to provide information to clients about local services and resources available for their use. The contractor will maintain information on the community in order to provide effective service delivery. 2.2.5 Market and advertise Family Support programs and services available to the Det 3 community to include monthly classes/workshops offered at the Spangdahlem Family Support Center. 2.2.6 Maintain brochures, books, magazines, etc., as a reference library for volunteers and customers. 2.2.7 Maintain the current welcome package and update as required to reflect new services, etc. 2.2.8 Present a briefing at newcomers functions detailing available family support programs and services. The DET 3 Commander must approve briefing materials, content and format. The contractor shall ensure that copies of the briefing are available as required. 2.2.9 Establish and maintain a customer email listing (voluntary participation only). 2.2.10 Provide pre-deployment briefings, sponsorship training, and key spouse training as required. 2.2.11 Interface with key spouses to provide current program information including updated newcomer lists with home email addresses as provided by customers. 2.2.12 Assist all walk-in clients in completing the necessary paperwork to check out items from the loan closet, ensuring all returned items are documented and condition recorded. 2.2.13 Identify and advertise for new volunteers, maintain volunteers' files and ensure the annotation of hours worked, training and awards received and amount of childcare services used. The contractor shall provide an updated list of all volunteers to the Volunteer Resource Program manager at the Spangdahlem AB FSC on a monthly basis. The contractor is required to organize the annual volunteer recognition ceremony. 2.2.14 Provide timely input to the Det 3 FSS and First Sergeant on nominations for Det 3 Volunteer of the Quarter and Volunteer of the Year programs. The contractor shall forward nominations to the Spangdahlem AB FSC Volunteer Resource Program manager for further action. 2.2.15 Provide or coordinate the delivery of Air Force Aid Society community programs such as "Bundles For Babies." 2.2.16 Monitor the Learning Resource Center to include, but not limited to the following duties: maintain sign-in/ sign-out sheets, install periodic updates to software, report systems problems to appropriate installation repair units and /or independent internet service provider (ISP), oversee customer activities to ensure compliance with regulations for appropriate use of government computer systems, and submit requests to Commander. 2.2.17 Assist the Spangdahlem AB Relocation Assistance Program staff in building the Det 1 area-specific information for the automated Standard Installation Topic Exchange Service, (SITES) database. The contractor shall coordinate quarterly with the Spangdahlem AB Relocation Assistance Program staff to update the database. 2.2.18 Coordinate with the Spangdahlem AB Relocation Assistance Program staff annually to update the Det 3 area-specific information used in Air Force Crossroads. 2.2.19 Coordinate with the Commander's Support Staff (CSS) to facilitate the ‘Smooth Move' briefing. 2.2.20 Coordinate/assist with organizers for local annual Spouse Appreciation Day (May). 2.2.21 Develop, track, and report outcome measures as developed by Family Matters Operations. The contractor shall prepare narrative and statistical reports (AF Form 2806) to identify program trends, usage, and accomplishments. Reports are to be provided to the Spangdahlem AB FSC GSU POC by the last duty day of each Fiscal Year quarter. 2.2.22 In process all new comers so as to assess the quality of the sponsorship program. Be a focal point along with sponsors on answering questions for inbound members. 2.2.23 Lead/assist Det staff setting up morale events such as annual Balls, monthly BBQs, food events, morale trips. 2.2.24 Assist US National Military Representative staff in setting up Town Hall events geared toward families. 2.2.25 Perform administrative duties as necessary. 3. QUALIFICATIONS: 3.1 A current resume and copies of transcripts must be submitted with the contractors offer showing compliance with this section of the statement of work. GOVERNMENT FURNISHED MATERIALS: The government will provide facilities, equipment, and supplies of a general nature sufficient to perform on the contract to include the following: 4.1 DET 3 MSS will provide: Office space, telephones with appropriate DSN and commercial capabilities, access to a Pentium computer with Microsoft Office software, printer, facsimile machine, access to.mil ("dot" mil) sites, dedicated e-mail account on the base LAN, basic office supplies, forms, publications, and reference materials directly related to Family Support programs and services from government or commercial sources. Access to copy machines/base reprographic facilities for reproduction of material directly related to Family Support. Contractor shall comply with applicable policies/regulations concerning the quantities and types of material reproduced. Access to base conference and training rooms. 5. WORK HOURS: 5.1 Services will normally be performed between 0800-1700, Monday through Friday. However, specific hours will be determined by needs of the community in coordination with Det 3 MSS and First Sergeant. The contractor is required to work an average of six (6) hours per day and 30 hours per week for 52 weeks, for a total of 1560 hours. Contractor may not exceed 40 hours in any given week. Contractors are not required to work Shape holidays. The following are a list of Shape holidays: New Year's Day (Observed) Good Friday Easter Monday Labor Day (Observed) Ascension Day Whit Monday Belgium National Holiday Assumption Day All Saints Day Remembrance Day Christmas Day (Observed) Boxing Day Days off and holidays are in accordance with local policy. Time off must be approved by the USAFE Det 3 MSS commander and shall not exceed 10 scheduled duty days per year without pay. The contractor will coordinate all leave, in writing, with the Det 3 Commander and courtesy coordinate with the Spangdahlem AB FSC. Coordination shall take place not later than 10 duty days prior to the planned absence. Leave is not paid for by the Government. 5.2 In the event of extended leave (more than 10 days) or emergency leave (more than 10 days), the contractor shall coordinate a proper and qualified replacement during the extended absence. Subcontracting may be allowed for no longer than 10 days of the Performance Period. The contractor shall coordinate in writing with the Det 3 Commander and courtesy coordinate with the Spangdahlem AB FSC. Subcontractors must meet or exceed the required qualifications, and must be able to manage all responsibilities in accordance with this SOW. 6. RESTRICTIONS: 6.1 The contractor is prohibited from receiving any moneys or benefit from any source as a direct or indirect result of the services provided to Family Support clients. 6.2 Confidentiality cannot be given to customers. However, any assistance provided is covered under the Privacy Act and should be treated accordingly. Related questions may be directed to the GSU POC at the Spangdahlem AB FSC.
