- Dry Ice Price Schedule This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is F1A4A03099A001. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-62. This requirement is set aside 100% for small businesses. The North American Industry Classification System (NAICS) code is 325120 with a 1000 employee size standard. Award will be based on Lowest Price Technically Acceptable (LPTA). Written proposals are required (oral offers will not be accepted). Only one contract will be awarded. LINE ITEMS: 0001: Delivery of Dry Ice Pellets. Period of Perfomance: 15 June 2013 - 31 December 2013. 1001: Delivery of Dry Ice Pellets. Period of Perfomance: 01 January 2014 - 31 December 2014. 2001: Delivery of Dry Ice Pellets. Period of Perfomance: 01 January 2015 - 31 December 2015. 3001: Delivery of Dry Ice Pellets. Period of Perfomance: 01 January 2016 - 31 December 2016. 4001: Delivery of Dry Ice Pellets. Period of Perfomance: 01 January 2017 - 31 December 2017. Required place of performance is located at Clinical Laboratory Shipping Department at Joint Base Anacostia-Bolling, DC. The resulting contract will be a firm fixed price. Statement of Work (SOW) for Dry Ice Delivery 1.0. DESCRIPTION OF SERVICES: The Contractor shall provide the 579th Medical Group (Joint Base Anacostia-Bolling, DC) with dry ice service. 1.1. Tasks: 1.1.1. The Contractor shall provide the following service: 1.1.1.1. Delivery of 120 lbs. of dry ice pellets to the Clinical Lab section in bldg. 1300. 1.1.1.2. Delivery will be every Friday morning with the exception of holidays, in which case it will be delivered the next duty day. 2.0. GOVERNMENT AND CONTRACTOR FURNISHED EQUIPMENT: 2.1. The government will provide storage container. 2.1.1. The government personnel will make arrangements for entrance of contractor onto Joint Base Anacostia-Bolling 2.1.2. Subject to security regulation, the government will permit access to the Clinical Lab section in bldg. 1300. 2.1.3 Government personnel will provide access to required areas. 3.0. GENERAL INFORMATION: 3.1. Hours of Operation: 0730 - 1630, Monday - Friday, exclusive of government holidays. 3.2. Location: 238 Brookley Avenue, Bldg. 1300. 3.3. Point of contacts. 3.3.1. Mr. Nicholas Didonato, Clinical Laboratory Supervisor, 202-767-5513. 3.4. Contractor shall provide a point of contact and telephone number to receive all request calls placed by the government. 3.5. If the Laboratory needs an Emergency delivery of dry ice the Contractor shall respond to each call within 24 hours of notification. 4.0. SECURITY REQUIREMENTS: 4.1. Contractor shall provide 2 forms of identification, social security number, birthdate and full name of employees to gain access to the installation. To operate a vehicle on the installation employees must also have current registration card and insurance on vehicles being operated. 4.2. Government will provide constant surveillance of contractor employees while at job site during the entire period of job performance. FAR 52.212-3 "Offeror Representations and Certifications - Commercial Items" applies to this acquisition and offeror(s) must include a completed copy with its offer.
